摘要
目的探讨肺部感染对急性缺血性脑卒中患者病情及预后的影响,为临床诊疗提供参考依据。方法选择2014年1月-2015年2月353例急性脑卒中患者为研究对象,选取60例急性脑卒中后并发肺部感染患者为观察组,60例未合并肺部感染者为对照组,两组均采用常规缺血性脑卒中治疗方案,观察组在此基础上应用抗感染治疗,分析急性脑卒中患者肺部感染的病原学分布情况,数据采用SPSS 17.0软件进行统计分析。结果 60例并发肺部感染患者共检出60株病原菌,以革兰阴性菌为主,共41株占68.33%;两组患者治疗后21d,NIHSS评分及MRS评分均较入院时明显降低,但对照组患者改善更加明显(P<0.05);观察组病死率为11.667%,显著高于对照组,差异有统计学意义(P<0.05)。结论肺部感染会导致急性缺血性脑卒中患者病情复杂,神经功能障碍加重,预后不佳。
OBJECTIVE To explore the influence of pulmonary infections on the illness condition and prognosis of the patients with acute ischemic stroke so as to provide guidance for clinical diagnosis and treatment .METHODS A total of 353 patients with acute ischemic stroke who were treated in the hospitals from Jan 2014 to Feb 2015 were recruited as the study objects ,then 60 acute ischemic stroke patients complicated with postoperative pulmonary in‐fections were assigned as the observation group ,while 60 patients without pulmonary infections were set as the control group .Both groups were treated with conventional treatment program for ischemic stoke ,and the observa‐tion group was given the additional anti‐infection treatment .The etiology of the pulmonary infections in the acute stroke patients was analyzed ,and the statistical analysis of data was performed with the use of SPSS 17 .0 soft‐ware .RESULTS Totally 60 strains of pathogens were isolated from the 60 patients complicated with pulmonary in‐fections ,of which 68 .33% (41 strains ) were gram‐negative bacteria .The NIHSS score and MRS score were sig‐nificantly lower on day 21 after the treatment than at the admission ,however ,the improvement of the control group was more remarkable (P〈0 .05) .The mortality rate of the observation group was 11 .667% ,significantly higher than that of the control group (P〈0 .05) .CONCLUSION The pulmonary infections may result in the com‐plicated illness condition of the patients with acute ischemic stroke and deterioration of neurological dysfunction as well as the poor prognosis .
