高血压左室肥厚患者心电图Cornell电压的测定及其与病情评估的关系 被引量:16

Measurement of ECG Cornell voltage in hypertension patients with left ventricular hypertrophy and its correlation with disease assessment

摘要 目的高血压左室肥厚患者心电图Cornell电压水平及其与病情的关系。方法选择经心脏彩超检查确诊的高血压左室肥厚患者作为观察组,选择健康体检者作为对照组,测定两组受试者的心电图Cornell电压、病情相关超声指标和血清指标。结果观察组中男性患者和女性患者的Cornell电压值和Cornell电压阳性率均高于对照组的男性和女性健康者;Cornell电压诊断男性左室肥厚(LVH)的特异性和灵敏性分别为96.15%和87.76%,诊断女性LVH的特异性和灵敏性分别为93.18%和89.73%。观察组患者的左心室舒张末期内径(LVEDD)、左心室后壁厚度(LVPWTd)高于对照组且与Cornell电压呈正相关,左心室射血分数(EF)和胱抑素C(Cys C)低于对照组且与Cornell电压呈负相关。结论高血压左室肥厚患者的Cornell电压值和阳性率均显著增加,与超声诊断的一致性高;Cornell电压值和病情指标具有良好的相关性、能够准确判断病情。 [ Objective ] To study the ECG Cornell voltage in patients with left ventricular hypertrophy (LVH) and its correlation with patient's condition. [ Methods ] Hypertension patients with left ventricular hypertrophy diagnosed by echocardiography were enrolled in observation group, healthy examination people were enrolled as control group. Then electrocardiogram Cornell voltage, disease related ultrasound and serum indicators in two groups were detected. [ Results ] Cornell voltage value and Cornell voltage positive rate of male and female patients were higher than those of male and female healthy subjects. Specificity and sensitivity of Cornell voltage for diagnosing male LVH were 96.15% and 87.76%, and 93.18%, 89.73% for diagnosing female LVH. LVEDD, LVPWTd and IMD of observation group were higher than those of control group and were positively correlated with the Cornel1 voltage. EF and CysC levels of observation group were lower and were negatively correlated with the Cornel1 vohage. [ Conclusion ] Coruell voltage value and positive rate of hypertension patients with left ventricular hypertrophy significantly increase and coincide with ultrasound diagnosis. Cornell voltage value has a good correlation with disease indexes and can accu- rately determine the patient's condition.
