摘要
目的:研究安络化纤丸对慢性乙型肝炎(CHB)患者肝纤维化和血流动力学的影响,探讨彩色多普勒超声在评价安络化纤丸疗效中的作用。方法:140例CHB患者按是否联合应用安络化纤丸分为治疗组(72例)和对照组(68例),比较两组患者治疗前和治疗12个月后的肝功能、肝纤维化和门静脉血流动力学指标的差异,并进行相关性分析。结果:1治疗前,两组患者的肝功能、肝纤维化、Child-Pugh分级、门静脉最大血流速度(Vmax)和高压指数(PHI)比较,差异均无显著性意义(p>0.05);2治疗12个月后,两组患者的肝功能、肝纤维化、Child-Pugh积分、Vmax和PHI均明显改善;与对照相比,治疗组改善更为明显,差异均有显著性意义(p<0.01);3治疗组患者Vmax和PHI改善值与肝功能、肝纤维化、Child-Pugh积分改善值呈正相关(p均<0.05)。结论:CHB患者经安络化纤丸治疗后,门静脉血流动力学改善与肝功能、肝纤维化改善具有较好的一致性,表明彩色多普勒超声对评价安络化纤丸的疗效具有一定的价值。
Objective：To assess the impacts of Anluo Huaxian capsule on the liver fibrosis and hemodynamic in patients with chronic hepatitis B（CHB）,and investigate the effect of color Doppler ultrasound on evaluating the efficacy of Anluo Huaxian capsule. Methods：According to whether combined Anluo Huaxian capsule,140 cases of CHB patients were divided into experimental group（72 cases）and control group（68cases）. The indexes of liver function,liver fibrosis and portal hemodynamic were compared before and 12 months after treatment in the two groups,and correlation analysis was performed. Results：1 Before treatment,liver function,liver fibrosis,Child-Pugh class,portal vein blood flow velocity（Vmax）and portal hypertension index（PHI）were similar in the two groups,the difference was not statistically significant（P〉0.05 for all）. 2 After 12 months treatment,there were significant improvement in the liver function,liver fibrosis,Child-Pugh class,Vmax and PHI in the two groups. However,compared with the control group,the improvements were more significantly in the experimental group,the difference was statistically significant（P〈0.01 for all）. 3 The correlations between the improvement of portal hemodynamic and the improvement of liver function,liver fibrosis and Child-Pugh class were significant positive correlation in the experimental group（P〈0.01 for all）. Conclusion：There are significant correlation between the improvement of portal hemodynamic and the improvement of liver function and liver fibrosis after the treatment of Anluo Huaxian capsule on CHB,suggesting that the color Doppler ultrasound has a good value for evaluation of efficacy of Anluo Huaxian capsule.
