Prevalence of infection with soil-transmitted nematodes in Yunnan Province

摘要 目的了解云南省土源性线虫人群感染现状。方法于2012-2014年对全省12个县采用整群随机抽样方法确定调查点,调查对象为3周岁以上常住居民。收集受检者粪便,采用Kato-Katz(一粪二检)法检查肠道蠕虫卵,分别计算蛔虫、鞭虫和钩虫每克粪便虫卵数(EPG),同时收集调查者的性别、年龄和受教育等情况。并将调查结果与1992年和2003年结果进行比较分析。抽取10户居民菜园、厕所周边、厨房、庭院的土样,采用改良饱和硝酸钠漂浮法查蛔虫卵。结果共调查24 870人,土源性线虫感染率为7.47%(1858/24870),与1992和2003年的19.3%和28.86%比较分别下降61.30%和74.12%。蛔虫、鞭虫、钩虫、绦虫和蛲虫感染率分别为1.21%(300/24870)、0.40%(99/24870)、6.01%(1494/24870)、0.004%(1/24870)和0.74%(2/271)。蛔虫、鞭虫和钩虫感染者的平均EPG分别为2 094、196和1082。男性和女性感染率分别为6.81%和8.07%,差异有统计学意义(χ2=14.219,P<0.05);各年龄组间感染率差异有统计学意义(χ2=429.14,P<0.01);不同职业和民族间感染率差异也有统计学意义(χ2职业=346.71,χ2民族=1 090.85,P均<0.01);不同文化程度人群感染率差异有统计学意义(χ2=46.71,P<0.01)。结论云南省人群土源性线虫感染率呈下降趋势,但在经济欠发达地区感染率仍较高,钩虫感染率下降趋势缓慢。 Objective To ascertain the prevalence of soil-transmitted nematodes in Yunnan Province. Methods Study sites in twelve counties of Yunnan Province were determined by clustered random sampling,and fecal samples were collected from residents age 3and above from 2012 to 2014.Feces were checked for the eggs of roundworms,hookworms,whipworms,or tapeworms using the Kato-Katz method.The number of eggs per gram(EPG)of feces was calculated.Results were compared to results from 1992 and 2003.Samples were also collected from vegetable gardens,around lavatories,in kitchens,and in courtyards of 10 randomly selected homes where occupants tested positive for eggs in their stool.Samples were examined for roundworm eggs using a modified floatation test with saturated sodium nitrate. Results In total,24 870 residents of 12 counties were examined.The overall rate of infection with soil-transmitted nematodes was 7.47%(1 858/2 4870),and this rate decreased 61.3%from the rate of infection in 1992 and 74.12%from the rate of infection in 2003.The prevalence of roundworms was 1.21%(300/24 870),the prevalence of whipworms was0.39%(99/24,870),the prevalence of hookworms was 6.01%(1 494/24 870),the prevalence of tapeworms was0.004%(1/24,870),and the prevalence of pinworms was 0.74%(2/271).The average EPG of feces was 2 094 for roundworms,196 for hookworms,and 1 082 for whipworms.The prevalence of infection differed in males and females(χ2=14.219,P0.05).Prevalence also differed among age groups(χ2=429.14,P0.01).Rates of infection also differed by occupation and ethnic group(χ2Occupation=346.71,χ2ethnic=1 090.85,P0.01).People with a higher level of education level had a lower prevalence of infection(χ2=46.71,P0.01). Conclusion The prevalence of soil-transmitted nematodes is decreasing in Yunnan Province but remains high in underdeveloped areas.The rate of hookworm infection is slowly declining.
