3.0T磁共振3D-TOF-MRA对颅内微小动脉瘤的诊断价值 被引量:8

摘要 目的探讨3.0T磁共振三维时空飞跃法磁共振血管成像(3D-TOF-MRA)对颅内微小动脉瘤(最大径小于或等于3.0mm)的诊断价值。方法对171例疑似动脉瘤或其他脑血管病患者行3.0T3D-TOF-MRA及三维数字减影脑血管造影(3DDSA)检查,以3D-DSA诊断结果为标准计算3D-TOF-MRA诊断微小动脉瘤的灵敏度、特异度及准确性。结果 171例受检患者,经3D-DSA证实,47例患有微小动脉瘤,共计56个,其中8例有蛛网膜下腔出血。3D-TOF-MRA共检出48例,共57个微小动脉瘤,假阳性2个,漏诊1个。59例患者3D-TOF-MRA及3D-DSA检查均为阴性。3D-TOF-MRA诊断微小动脉瘤的灵敏度、特异度、准确性分别为96.6%(56/58)、98.3%(59/60)、97.5%(115/118)。结论 3.0T3D-TOF-MRA尤其是容积重建技术对颅内微小动脉瘤诊断具有较高价值,基本可以替代3D-DSA,成为筛查及诊断颅内微小动脉瘤的首选无创性检查。
