摘要
目的:比较青海省过马营镇中心卫生院医改前后住院患者住院医疗费用负担情况,为进一步研究减轻偏远地区农牧民住院负担提供依据。方法:通过收集医改前后4个年度参加新农合农牧民在卫生院住院资料、查阅统计年鉴,比较分析卫生院住院就诊人次、次均费用和住院患者经济负担变化情况。结果:医改前后卫生院的软、硬件得到了较大的改善,住院服务能力明显提高。2014年比2009年住院患者出院人次数增加1.35倍,医技科室检查住院人次增加1.5倍以上,次均住院费用增加0.24倍,住院医疗总费用增加1.81倍,住院基金支出额增加1倍,住院患者自付医疗费用增加3.01倍,住院费用实际报销比由59.70%降到42.41%。次均住院自付费用增加158.66元。结论:医改加强了卫生院的服务能力,但住院费用呈增加趋势,农牧民住院负担加重,应综合考虑新农合制度发展的影响因素,加强制度建设,确保新农合制度可持续发展。
Objective To compare the hospitalization cost burden on farmers and herdsmen in one township health center of Qinghai before and after the healthcare reform, and provide references for decreasing their hospitalization cost. Methods The data of hospitalization cost in township health center in four years was collected. And the statistical yearbook was searched to comparatively analyze their burden of hospitalization cost. Results The hardware and software and capability of hospitalized services of this town- ship health center had improved greatly. Compared with those in 2009, the person-time number of leaving-inpatients increased by 2.35 times, person-time number of being examined inpatients increased by 2.5 times, per-time hospitalization cost increased by 1. 24 times, total hospitalization cost increased by 2.81 times, expenditure from hospitalization funds increased by 2 times, inpatients＇ out-of-pocket medical expenses increased by 4. O1 times, actual reimbursement ratio of hospitalization expenses reduced by 17. 29% ,and per-time out-of-pocket expenses increased 158.66 Yuan RMB in 2014. Conclusion The capability of medical services of township health centers had been improved after the new medical reform. But the burden of hospitalization cost on farmers and herdsmen had also increased. It is suggested to study the factors influencing the development of new rural cooperative medical sys- tem （NCMS）, and strengthen system construction to ensure the sustainable development of NCMS.
