骨密度及相关生化指标在系统性红斑狼疮继发骨量减少及骨质疏松中的临床应用 被引量:3

Clinical application of bone mineral density and relevant biochemical indexes in secondary osteopenia and osteoporosis due to systemic lupus erythematosus

摘要 目的 探讨骨密度(BMD)和血钙、血磷、25(OH)维生素D3、碱性磷酸酶(ALP)、Ⅰ型前胶原氨基端伸展肽(PINP)、Ⅰ型胶原羧基末端肽(CTX)等指标在系统性红斑狼疮(SLE)继发骨量减少及骨质疏松中的临床价值。方法 通过测定SLE女性患者腰椎(L1~4)、两侧股骨颈BMD,从中筛选出40例继发骨量减少及骨质疏松患者,测定血钙、血磷、25(OH)维生素D3、ALP、PINP、CTX等指标,并与40例同年龄段健康女性各指标进行对比。结果 所选患者与同年龄段健康女性相比各部位BMD均显著降低(P〈0.001)。患者的25(OH)维生素D3水平较健康女性显著降低(P〈0.001),而血钙、血磷、ALP、PINP、CTX等差异无统计学意义(P〉0.05)。结论 SLE继发骨量减少及骨质疏松患者的血清25(OH)维生素D3水平下降,血钙、血磷、ALP、PINP、CTX水平与BMD不存在显著相关性。 Objective To explore the clinical value of bone mineral density( BMD) and serum calcium、serum phosphate、25( OH) vitamin D3、alkaline phosphatase( ALP) 、aminoterminal propeptide of type Ⅰ procollagen( PINP),type Ⅰ collagen carboxyl terminal peptide( CTX) levels in patients with secondary osteopenia and osteoporosis due to systemic lupus erythematosus( SLE).Methods Forty patients with secondary osteopenia and osteoporosis by detecting SLE female patients' BMD of lumbar spine( L1 ~ 4) and bilateral femoral neck were selected. Serum calcium、serum phosphate、25( OH) vitamin D3,ALP,PINP,CTX were detected and compared with forty healthy women of the same age. Results Each part of BMD in patients were significantly lower than that in healthy women of the same age( P 0. 001). The 25( OH) vitamin D3 level in patients were also significantly lower than healthy women of the same age( P 0. 001),but the serum calcium,serum phosphate,ALP,PINP,CTX level had no significant differences( P 0. 05). Conclusion The 25( OH) vitamin D3 level decreases in patients with secondary osteopenia and osteoporosis due to SLE,and there is no significant correlation between the levels of serum calcium,serum phosphate,ALP,PINP,CTX and BMD.
