摘要
目的分析应用连续性肾脏替代疗法(CRRT)技术实施亚低温治疗重型颅脑损伤的临床应用效果。方法我院2014年1~12月收治重度颅脑损伤患者16例为观察组,应用常规治疗结合CRRT技术实施亚低温治疗,2013年3~12月重度颅脑损伤患者14例作为对照组,采用常规治疗。比较两组GCS评分、颅内压、患者血糖及血乳酸水平预后情况。结果观察组3个月内死亡率25.00%,低于对照组死亡率71.43%,差异有统计学意义(x2=6.467,P〈0.05);观察组术后24、48、72h颅内压均较对照组低(均P〈0.05);观察组血糖、乳酸水平均低于对照组(均P〈0.05)。结论应用CRRT实施亚低温治疗可安全有效降低重度颅脑损伤患者高颅内压,改善高血糖反应及高乳酸血症,降低死亡率。
Objective To analyze the clinical effect of continuous renal replacement therapy(CRRT) technology implementation hypothermia treatment of severe traumatic brain injury. Methods 16 case of patients with severe traumatic brain injury from January 2014 to December 2014 in our hospital were selected for the observation group,who had been treated with the application of conventional therapy combined with hypothermia CRRT technology implementation.14 case of patients with severe traumatic brain injury from March 2013 to December 2013 were selected for the control group which was only treated with conventional treatment. GCS score, intracranial pressure, blood glucose and blood lactate levels prognosis of the two groups were compared. Results The mortality rate within 3 months of observation group was lower than the control group(25.00% vs. 71.43%), the difference was statistically significant(x2= 6.467, P〈0.05); The 24, 48 and 72 hours after surgery intracranial pressure of the observation group were lower than the control group(all P〈0.05); The blood glucose, lactate levels of the observation group were lower than the control group(P〈0.05). Conclusion CRRT implementation of hypothermia therapy is safe and effective in patients with severe traumatic brain injury,It can reduces high intracranial pressure and can improve hyperglycemia and lactic acidosis and reduce mortality.
