摘要
目的探讨胱抑素C(Cys C)、同型半胱氨酸(HCY)、B型钠尿肽(BNP)、超敏C-反应蛋白(hs-CRP)对早期糖尿病患者肾损害的临床诊断价值。方法根据尿白蛋白排泄率(UAER)将150例2型糖尿病(T2DM)患者分为3组,分别为单纯糖尿病组(SDM组,n=53)、早期糖尿病肾病组(EDN组,n=49)、临床糖尿病肾病组(CDN组,n=48)。另设100例健康体检者为正常对照组(NC组)。分别检测血清Cys C、HCY、BNP、hs-CRP、尿素氮(BUN)及肌酐(Cr),并比较各指标的组间差异,分析其相关性。结果糖尿病肾病组Cys C、HCY、BNP、hs-CRP水平高于对照组,差异有统计学意义(P<0.05)。SDM组、EDN组、CDN组的Cys C、HCY、BNP、hs-CRP、UAER逐渐递增,3组间差异有统计学意义(P<0.05),且四项指标与UAER呈正相关(r值分别为0.623、0.518、0.571、0.683,P<0.05)。结论血清Cys C、HCY和BNP、hs-CRP含量随糖尿病肾病的发生以及严重程度逐渐增高,可作为早期诊断糖尿病肾病较敏感的指标,其联合检测对糖尿病肾病的预防、延缓或减慢疾病的进展有重要的意义。
Objective To investigate the clinical diagnostic value of combination detection of serum cystatin C (CysC) ,homocysteine (HCY) with B-type natriuretic Peptide (BNP) , high sensitive C reaction protein (hs-CRP) in early diabetes with renal injury. Methods According to urinary albumin excretion rate (UAER) , 150 patients with type 2 diabetes mellitus (T2DM) were divided into three groups : simple diabetes mellitus group ( SDM group, n = 53 ), early diabetic nephropathy group (EDN group, n =49),diabetic nephropathy group (CDN group, n = 48 ), the other 100 healthy subjects were served as normal control group (NC group). The serum levels of CysC, HCY, BNP, hs-CRP, blood urea nitrogen (BUN) and creatinine (Cr) were detected and compared among groups. Results The serum levels of CysC, HCY, BNP, hs-CRP in CDN group were significantly higher than those in NC group ( P 〈 0.05 ). There were significant differences in serum levels of CysC,HCY, BNP, hs-CRP among SDM group, EDN group, CDN group (P 〈 0. 05), with a gradual increasing trend, furthermore,the four indexes were positively correlated to UAER ( r = 0. 623, 0. 518, 0. 571,0. 683, respectively, P 〈 O. 05 ). Conclusion The serum levels of CysC, HCY, BNP, hs-CRP are gradually increased with the pathogenesis and severity of diabetic nephropathy, thus, these indexes can be regarded as sensitive indicators in early diagnosis of diabetic nephropathy. Furthermore the combination detection of these indexes plays an important role in preventing, delaying or slowing down the progress of diabetic nephropathy.
