罗哌卡因复合喷他佐辛用于儿童臂丛神经阻滞与术后镇痛的效果

Efficacy of anesthesia and postoperative analgesia by ropivacaine combined with pentazocine for bra- chial plexus block in children

摘要 目的观察罗哌卡因复合喷他佐辛用于腋路臂丛神经阻滞的手术麻醉和术后镇痛效果。方法选择肱骨髁上骨折拟行急诊手术患儿60例,随机分为复合组(0.25%罗哌卡因1ml/kg加喷他佐辛0.5mg/kg)和单药组(0.25%罗哌卡因1ml/kg)。记录麻醉起效时间、感觉阻滞效果、感觉阻滞持续时间、镇痛持续时间及不良反应。结果与单药组比较,复合组感觉阻滞持续时间(10.5±2.2hvs6.4±1.8h)和镇痛持续时间(19.1±2.8hvs14.2±2.0h)明显延长(P〈0.05)。但两组麻醉起效时间、感觉阻滞效果及不良反应差异无统计学意义(P〉0.05)。结论罗哌卡因复合喷他佐辛腋路臂丛神经阻滞可为肱骨髁上骨折患儿提供良好的手术麻醉和术后镇痛效果。 Objective To investigate the anesthesia and postoperative analgesia effects of the axillary brachial plexus block using ropivacaine combined with pentazocine in children. Methods Sixty children undergoing emergency operation of humeral supracondylar fracture were randomly allocated to mixed group (0.25%ropivacaine 1 ml/kg and 0.5 mg/kg pentazocine) and single group (0.25%ropivacaine 1 ml/kg). The onset time of block, effectiveness of block, duration of sensory block and postoperative analgesia, and side effects were recorded. Results Compared with the single group, the duration of sensory block and postoperative analgesia were prolonged significantly in mixd group (P〈 0.05). There were no statistical significance between the two groups about the onset time and effectiveness of block (P〉 0.05). Conclusion The axillary brachial plexus block using ropivacaine combined with pentazocine can provide good surgical anesthesia and postoperative analgesia for children with supracondylar fracture.
