摘要
目的：探讨无覆盖带涤纶套的深静脉导管出口护理在血液透析患者中的应用效果。方法选择2014年6月~2015年3月在本院行维持性血液透析且留置深静脉导管的患者60例，将其随机分为观察组和对照组，各30例。对照组患者导管出口处覆盖无菌纱布或一次性敷贴；观察组患者导管出口处不覆盖纱布或敷贴，观察两组导管相关感染发生率，评价血液透析的舒适度，统计平均护理用时及透析费用，并进行组间比较。结果观察组导管出口处感染、隧道感染、相关血流感染的发生率与对照组比较，差异无统计学意义（P〉0.05）；观察组患者血液透析舒适度明显优于对照组，差异有统计学意义（P〈0.05）；观察组平均护理用时短于对照组，一次透析费用低于对照组，差异有统计学意义（P〈0.05）。结论维持性血液透析深静脉出口无覆盖式护理也可以预防导管相关感染，且患者舒适度高，护理工作量和透析成本低，有较高的应用价值。
Objective To explore the application effect of deep venous catheter nursing of no coverage with polyester sheath in patients with hemodialysis. Methods 60 patients with needing hemodialysis and deep venous catheter in our hospital from Jun 2014 to March 2015 were selected and randomly divided into observation group and control group,30 cases in each group.The control group was given sterile gauze or one-time patches on the catheter exit,the observation group was given no sterile gauze or one-time patches on the catheter exit.The incidence of catheter related infections of two groups were observed,and comfort degree was evaluated.The average nursing time and expenditure hemodialysis was recorded and compared between two groups. Results The incidence of catheter exit and tunnel infection, associated bloodstream infections in observation group compared with control group,the difference was no statistically significant （P〉0.05）.The comfort degree of hemodialysis in observation group wa significantly better than that in control group,the difference was statistically significant （P〈0.05）.The average nursing time in observation group was shorter than that in control group,and the cost of one hemodialysis was lower than that in control group,the differences were significant （P〈0.05）. Conclusion Deep venous catheter nursing of no-coverage with polyester sheath in patients with maintenance hemodialysis can also prevent catheter related infections,with high comfort degree,low nursing workload and dialysis cost,has higher application value.
