摘要
目的了解儿童咽拭子标本中A群乙型溶血性链球菌的感染情况及药物敏感性,研究A群乙型溶血性链球菌菌株间相关性及型别组成。方法对采集的414份儿童咽拭子标本进行A群乙型溶血性链球菌的分离培养鉴定,采用K-B琼脂扩散法检测其药物敏感性,利用PFGE方法分型聚类。结果 414份标本中检出35株A群乙型溶血性链球菌,检出率为8.45%。男性阳性率为11.27%;女性阳性率为5.47%,不同性别阳性率差异有统计学意义(P<0.05)。不同时间和不同年龄组检出率差异无统计学意义(P>0.05)。35株菌对氯霉素、头孢吡肟、奥复星、头孢噻肟高度敏感,对大环内酯类药物高度耐药;35株菌主要分成14个PFGE型,优势带型有2种。结论本地区10岁以下年龄组儿童存在一定A群乙型溶血性链球菌感染率。头孢吡肟、奥复星、头孢噻肟可作为治疗的首选药物,分离的菌株表现出遗传多样性,但部分有相关性。
Objective Understand the infection and drug sensitivity of group AB-hemolytic Streptococcus in the throat swab specimens from children,and study on the correlation among strains of group AB-hemolytic Streptococcus strains and type composition. Methods 414 throat swab specimens from children in Dongcheng district were collected. All of these samples were used to isolate and group AB-hemolytic Streptococcus and identifiy. K-B agar diffusion method was used to detect the drug sensitivity,and the PFGE method was used to cluster. Results 35 strains of group AB-hemolytic Streptococcus were isolated from 414 specimens,with the detection rate of 8. 45%. The positive rate of male was 11. 27% and the positive rate of female was 5. 47%,the difference of detection rate between different genders was statistically significant（ P〈0. 05）. No significant difference was found in different time and different age groups（ P〉0. 05）. 35 strains were highly sensitive to chloramphnicol,cefepime,ofloxacin and cefotaxime and highly resistant to macrolides. 35 strains were divided into 14 PFGE patterns,2 of which are predominance. Conclusion Children under age 10 were generally susceptible to group AB-hemolytic Streptococcus in our region. Cefepime,ofloxacin,and cefotaxime should be the first choice. The separated stains were genetic diversity,but some strains showed similarity.
