表皮细胞生长因子冻干粉和阿维A辅助物理光疗法治疗银屑病的效果 被引量:6

Effect of epidermal growth factor freeze-dry powder and Acitretin assisted NB-UVB in the treatment of psoriasis

摘要 目的探讨表皮细胞生长因子(EGF)冻干粉和阿维A辅助窄谱中波紫外线(NB-UVB)物理光疗法治疗银屑病的效果。方法选取2011年1月~2014年12月在北京军区总医院诊治的130例银屑病患者,按照随机数字表法分成观察组(65例)和对照组(65例)。对照组仅给予NB-UVB物理光疗,观察组则联合EGF冻干粉以及阿维A药物治疗,比较两组的皮损改善情况、生活质量及相关细胞因子的表达水平。结果观察组的总有效率[92.31%(60/65)]明显高于对照组[76.92%(50/65)],差异有统计学意义(P〈0.05)。观察组治疗2、4、8周后的皮损面积和严重程度指数评分均低于对照组[(16.50±3.18)、(11.25±3.05)、(5.82±2.71)比(17.45±3.20)、(13.46±3.11)、(7.55±2.90)分],差异有统计学意义(P〈0.05)。观察组治疗2、4、8周后的皮肤病生活质量指数评分均低于对照组[(14.51±3.78)、(9.14±2.25)、(3.95±1.12)比(16.07±3.52)、(11.03±2.18)、(5.08±0.81)分],差异有统计学意义(P〈0.05)。观察组治疗后的EGF和T细胞α亚族趋化因子表达水平均低于对照组[(135.02±28.15)比(198.35±30.47)pg/m L,(45.38±13.60)比(102.75±41.20)pg/m L],差异有统计学意义(P〈0.05)。结论 EGF冻干粉和阿维A辅助物理光疗法治疗银屑病可以有效改善皮损,调节相关因子表达,促进皮肤改善,提高患者的生活质量。 Objective To investigate the effect of epidermal growth factor(EGF) freeze-dry powder and Acitretin assisted narrow-band ultraviolet B(NB-UVB) in the treatment of psoriasis. Methods 130 cases of psoriasis patients admitted to General Hospital of Beijing Military Command from January 2011 to December 2014 were selected and randomly divided into observation group(n=65) and control group(n=65) by the table of random number. Control group was given NBUVB only, and observation group was given EGF freeze-dry powder and Acitretin on the basis of the control group. The changes of skin lesions, the dermatology life quality and the expression of related cytokines between two groups were compared. Results Total efficiency rate in observation group was higher than that in control group [92.31%(60/65) vs76.92%(50/65)], the difference was statistically significant(P〈0.05). After treatment of 2, 4, 8 weeks, PASI score in observation group were lower than those in control group [(16.50 ±3.18),(11.25±3.05),(5.82±2.71) vs(17.45±3.20),(13.46±3.11),(7.55±2.90) points], the differences were statistically significant(P〈0.05). After treatment of 2, 4, 8 weeks, DLQI score in observation group were lower than those in control group [(14.51 ±3.78),(9.14 ±2.25),(3.95 ±1.12) vs(16.07 ±3.52),(11.03±2.18),(5.08±0.81) points], the differences were statistically significant(P〈0.05). After treatment, the expression levels of EGF and I-TAC in observation group were lower than those in control group [(135.02±28.15) vs(198.35±30.47) pg/m L,(45.38±13.60) vs(102.75±41.20) pg/m L], there were statistically significant differences(P〈0.05). Conclusion EGF freeze-dry powder and Acitretin assisted NB-UVB in the treatment of psoriasis can significantly relieve the lesions of psoriasis, regulate the expression of related cytokines, and improve the psoriasis patient′s quality of life.
