摘要
目的:探讨枸橼酸咖啡因和氨茶碱对早产儿呼吸暂停临床预后及脑神经发育的临床疗效。方法:随机选取笔者所在医院2013年1月-2014年1月收治的存在呼吸暂停的早产儿92例为研究对象,男41例,女51例,随机分成观察组和对照组,分别使用枸橼酸咖啡因和氨茶碱两种药物进行对比治疗。结果:治疗后,两组患者的血浆β-EP水平比较差异有统计学意义(P<0.05);观察组的呼吸暂时停止时间及用药时间均显著低于对照组,两组比较差异均有统计学意义(P<0.05);观察组治愈率为87.0%(40/46),死亡率为13.0%(6/46),对照组分别为82.6%(38/46)、17.4%(8/46),两组比较差异均无统计学意义(P>0.05);两组患儿治疗前后运动发育指数和智力发育指数比较差异均有统计学意义(P<0.05);治疗后,观察组患儿运动发育指数和智力发育指数均明显高于对照组,两组比较差异均有统计学意义(P<0.05)。结论:氨茶碱和枸橼酸咖啡因都能够纠正早产儿AOP现象,全面改善患儿预后,与氨茶碱相比,枸橼酸咖啡因可以更好地改善患儿的运动和神经发育。
Objective: To discuss the clinical effects of caffeine citrate and aminophylline on premature apnea, clinical prognosis and brain development.Method: 92 cases of premature infants with apnea were randomly selected as the research objects from January 2013 to January 2014 in our hospital, male of 41 cases, female of 51 cases, and which were divided into the observation group and the control group, they were used Caffeine Citrate and Aminophylline to contrast the treatments.Result: The plasma levels of β -EP in the observation group was significantly higher than those in the control group after treatment, and the difference was statistically significant(P〈0.05).The temporarily stop breathing time and medication time of the observation group were significantly lower than the control group, there were significant differences between the two groups(P〈0.05).The cure rate was 87.0%(40/46), the mortality was 13.0%(6/46), and the control group were 82.6%(38/46), 17.4%(8/46), there were no significant difference between the two groups(P〉0.05).After treatment, the motor development index and mental development index of the two groups were better than pre-treatment, the differences were statistical significance(P〈0.05).The motor development index and mental development index of the observation group were significantly higher than those in the control group, the differences were statistically significant(P〈0.05).Conclusion: Aminophylline and citrate caffeine both can correct the premature phenomenon of AOP and improve the prognosis of patients comprehensively, compared with aminophylline, caffeine citrate can better improve the premature infants's movement and neural development, so citrate caffeine is worthy of further promotion.
