摘要
目的:研究分析肉桂颗粒对椎基底动脉供血不足性眩晕患者血流动力学的影响,为临床治疗提供理论依据。方法:回顾性分析于2013年1月~2014年3月我院收治并经临床检查确诊为椎基底动脉供血不足性眩晕患者共64例,按双盲、随机原则分为2组,每组32例。其中治疗组采用肉桂颗粒治疗,对照组行安慰剂(玉米淀粉颗粒)治疗。监测两组患者治疗前及治疗后3天、5天、7天及14天的血流动力学指标。结果:治疗前与对照组相比,MAP、CI、SV、SI和PAWP等值均明显降低,统计学分析,差异具有统计学意义(P〈0.05)。治疗5天后,治疗组患者眩晕症状均有明显改善。血流动力学方面,治疗组患者治疗后的平均动脉压(MAP)、心脏指数(CI)、每搏输出量(SV)、每搏指数(SI)和肺动脉嵌压(PAWP)明显高于治疗前,有统计学差异(P〈0.05)。心率(HR)、体循环阻力(SVR)和肺循环阻力(PVR)明显低于治疗前,有统计学差异(P〈0.05)。14天后除PAWP外,其余指标两组相比差异均无统计学意义(P〉0.05)。治疗组的HR及SVR在14天后仍显著高于对照组,差异有统计学意义(P〈0.05),两组PVR相比差异无统计学意义(P〉0.05)。结论:肉桂颗粒能显著改善椎基底动脉供血不足性眩晕患者症状并有助于此类患者血流动力学的恢复,值得临床推广使用。
Objective: To study the hemodynamic effect of cinnamon granules on vertebral basilar artery insufficiency vertigo patients,and to provide a theoretical basis for the clinical diagnosis and treatment. Methods: 64 cases with vertebral basilar artery insufficiency vertigo in our hospital from January 2013 to March 2014 were randomly divided into two groups. The control group was treated by placebo( Corn Starch granules),while the treatment group was treated by traditional Chinese medicine Cinnamon granules. Then compare the blood stream dynamics after the treatment of 3 d,5 d,7 d and 14 d in two groups. Results: Before treatment,compared with the control group,MAP,CI,SV,SI and PAWP were significantly lower,and according to the statistical analysis the difference was statistically significant( P〈0. 05). After 5 days,patients with vertigo in the treatment group were improved. According to the hemodynamics in the treatment group the mean arterial pressure( MAP),cardiac index( CI) output,stroke volume( SV),stroke index( SI) and pulmonary artery pressure( PAWP) among patients after treatment were significantly higher than those before treatment,and there were differences in statistical analysis( P〈0. 05). Heart rate( HR) and systematic resistance( SVR) and pulmonary resistance( PVR) were lower than those before treatment,and it had a significant difference according to the statistical analysis( P〈0. 05). After 14 days,besides PAWP,other indexes had no statistically significant differences compared between two groups( P〈0. 05). As to HR and SVR,the treatment group was significantly higher than the control group after 14 d,and the difference was statistically significant( P〈0. 05); As to PVR,there was no statistically significant difference( P〈0. 05). Conclusion: Cinnamon granules can significantly improve the symptoms of vertebral basilar artery insufficiency vertigo and it's good for the recovery of hemodynamics among patients,which is worth clinical promotion.
