惊厥性脑损伤与白细胞介素-6、神经元特异性烯醇化酶的相关性研究 被引量:7

摘要 目的探讨惊厥性脑损伤患儿血清中白细胞介素-6(IL-6)和神经元特异性烯醇化酶(NSE)质量浓度变化的临床意义。方法观察惊厥住院患儿100例,根据病情分为惊厥持续状态组30例、多次惊厥组32例,单次惊厥组38例,选择同期无惊厥住院患儿30例作为对照组,采用电化学发光法测定患儿惊厥后24 h和72 h血清IL-6、NSE的质量浓度变化。结果惊厥持续状态组、多次惊厥组、单次惊厥组血清中IL-6和NSE质量浓度均明显高于对照组,差异有统计学意义(P<0.05);血清中IL-6和NSE质量浓度惊厥持续状态组明显高于多次惊厥组,多次惊厥组明显高于单次惊厥组,差异有统计学意义(P<0.05)。72 h后各惊厥组血清中IL-6和NSE质量浓度明显降低,差异有统计学意义(P<0.05)。直线相关回归分析显示:IL-6与NSE呈正相关(r=0.81,P<0.05)。结论 IL-6与NSE参与了惊厥性脑损伤的病理损害过程,能反映惊厥性脑损伤病理损害的严重程度,评估惊厥患儿的预后。 Objective To explore the clinical significance of changes of serum interleukin-6 (IL-6) and neuron-specific enolase (NSE) mass concentrations in children with seizure-induced brain injury. Methods One hundred children with seizure-induced brain injury were divided into status eonvulsivus group (30 children) , repeated convulsion group (32 children) , and single seizure group (38 children) according to the children's condition; 30 hospitalized children without seizure were selected as control group; electrochemical lumines- cence method was used to detect the changes of mass concentrations of serum IL-6 and NSE after 24 and 72 hours of seizure. Results The mass concentrations of serum IL-6 and NSE in status convulsivus group, repeated convulsion group, and single seizure group were statistically significantly higher than those in control group (P〈0.05) ; the mass concentrations of serum IL-6 and NSE in status convulsivus group were statistically significantly higher than those in repeated convulsion group ( P〈0.05 ) ; the mass concentrations of serum IL-6 and NSE in repeated convulsion group were statistically significantly higher than those in single seizure group ( P〈0. 05 ) . After 72 hours, the mass concentrations of serum IL-6 and NSE in three seizure groups decreased significantly, there were statistically significant differences (P〈0. 05 ). Linear correlation regression analysis showed that IL-6 was positively correlated with NSE (r = 0. 81, P〈0. 05 ) . Conclusion IL-6 and NSE are involved in the pathological process of seizure-induced brain injury, which can reflect the serious degrees of pathological injuries and evaluate the prognosis.
