老年糖尿病视网膜病变血胱抑素C与糖尿病肾病相关性研究 被引量:4

A correlative study between serum cystatin C and diabetic nephropathy in elderly patients with diabetic retinopathy

摘要 目的探讨老年2型糖尿病(T2DM)不同视网膜病变(DR)分期患者血清胱抑素C(Cys-C)水平变化及与糖尿病肾病(DN)的相关性。方法选取住院治疗的老年T2DM患者125例,进行眼底照相或眼底荧光造影,根据2002年国际分级标准将其分增殖期糖尿病视网膜病变(PDR)组35例、背景期糖尿病视网膜病变(BDR)组41例、老年2型糖尿病无DR病变(NDR)组49例,老年健康对照(CON)组30例。各组分别检测血Hb A1c、Cys-C,24 h尿白蛋白排泄率(UAER),分析各组的特点及相关因素。结果 Cys-C、UAER在CON组与NDR组间差异无统计学意义(P>0.05),与NDR相比,Hb A1c、Cys-C、UAER在PDR组及BDR组均显著升高,差异均有统计学意义(P<0.05),且随DR的进展呈增长趋势,PDR组较BDR组升高更显著,差异均有统计学意义(P<0.05),且Hb A1c与Cys-C呈正相关(P<0.05),UAER与Cys-C呈正相关(P<0.05)。结论老年糖尿病视网膜病变和尿白蛋白排泄率存在一定相关性,Cys-C有可能成为预测老年DR发生发展的生物学指标。 Objective To study the relationship between serum cystatin C and diabetic nephropathy (DN)in elderly patients with diabetic retinopathy(DR). Method 125 cases of elder type 2 diabetes patients in our hospital from January 2013 to April 2014 were investigated to have a fundus examination and fundus fluorescence angiography. According to 2002 international classification standard,35 patients were in proliferative DR (PDR) group, 41 in' backdround DR (BDR) group,49 in 2 DM without DR (NDR) group,and 30 healty elder volunteers as control group. Clinical courses were monitored, while glycated hemoglobin( HbA1 c) , Cys-C and Urinary albumin excretion rate(UAER) were determined. Result Compared with control group, serum Cys-C and UAER in NDR group were no difference (P 〉 0.05). Compared with NDR group, the serum levels of Cys-C, HbA1 c and UAER in PDR and BDR group were significantly incresed (P 〈 0.05 )and the increse were more obviously in PDR group than that in BDR group( P 〈 O. 05 ). There were positive correlations between Cys-C and HbAl c(P 〈 O. 05). There were positive correlations between serum Cys-C and UAER ( P 〈 0.05 ). Conclusions Diabetic retinopathy is closely correlated with UAER in elder patients with type 2 diabetes. Cys-C and HbAlc may serve as biological indicators to predict the occurrence and development of DR.
