磁共振LAVA动态增强半定量分析对前列腺癌的诊断价值 被引量:16

摘要 目的通过磁共振LAVA动态增强(DCE-MRI)相关参数的分析,探讨DCE-MRI对前列腺癌的诊断价值。方法对经组织病理学证实的29例PCa和58例BPH患者行肝脏快速容积采集序列(LAVA)多期动态增强扫描,获得SI-T曲线,并计算出峰值时间(Tmax)、最大信号强度(Slmax%)、强化率(R)、流出分数(washout),比较各参数在PCa、g BPH、s BPH之间的差异,并进行ROC曲线分析。结果 PCa组SI-T曲线以速升下降型为主,g BPH、s BPH组以平台型为主;Tmax、R、Washout三组组间差异有统计学意义(P<0.05),Slmax%组间差异无统计学意义(P>0.05);Tmax、R、Washout值g BPH与s BPH分组间差异无统计学意义(P>0.05);Tmax的ROC曲线下面积最大,诊断敏感度及特异度分别为93.6%、84.6%;PCa组Tmax多在1分钟内(75.9%),s BPH组Tmax多在1-2分钟内(55.5%),而g BPH组Tmax多在两组区间,即1-2分钟(42.5%)及2分钟后(40.0%)。结论 DCE-MRI半定量分析在前列腺癌诊断中具有较高价值,可用于前列腺良恶性病变的鉴别诊断。 Objective By analyzing the LAVA dynamic enhanced magnetic resonance (DCE-MRI) related parameters to evaluate the diagnostic value of DCE-MRI for prostate cancer. Methods 29 patients with PCa and 58 BPH patients with liver histopathology confirmed the line fast volume acquisition sequence (LAVA) nmhiphase dynamic enhanced scan, access SI-T curve, and calculate the peak time (Tmax), the maximum signal strength (Slmax %), enhancement ratio (R), out of scores (washout), compare the differences in the parameters between PCa, gBPH, sBPH of, and P.OC curve analysis. Results PCa group SI-T curve at a speed drop liter main type, gBPH, sBPH platform-type-based group; between Tmax, R, Washout three groups was statistically significant (P〈0.05) differences between the groups was not Slmax % statistically significant (P〉0.05); Tmax,R Washout value between gBPH and sBPH group showed no significant difference (P〉0.05); area under the ROC curve Tmax largest diagnostic sensitivity and specificity were 93.6%, 84.6%; PCa group within Tmax more (75.9%) one minute, sBPH group more in Tmax (55.5%) for 1-2 minutes, and two more in gBPH group Tmax range, ie 1-2 minutes (42.5%) and after 2 minutes (40.0%). Conclusion Senti-quantitative analysis of DCE-MP.I has a higher value in the diagnosis of prostate cancer can be used for differential diagnosis of benign and malignant prostate lesions.
