摘要
目的探讨空心螺钉内固定联合带旋髂深血管髂骨瓣移植治疗PauwelsⅢ型青壮年股骨颈骨折的手术方法及疗效。方法对18例PauwelsⅢ型青壮年股骨颈骨折患者采取空心螺钉内固定联合带旋髂深血管髂骨瓣移植术治疗,观察效果。结果 18例全部获得随访,16例术后骨折顺利愈合,未出现股骨头坏死,髋关节功能恢复良好。2例晚期出现股骨头缺血坏死,行人工全髋关节置换。结论空心螺钉内固定联合带旋髂深血管髂骨瓣移植术是治疗PauwelsⅢ型青壮年股骨颈骨折的有效方法 ,值得临床推广应用。
Objective To investigate surgical method and curative effect of hollow screw internal fixation combined with deep iliac circum flex blood vessels iliac flap graft in the treatment of Pauwels Ⅲ type femoral neck fractures in young adults. Methods A total of 18 young adults patients with Pauwels Ⅲ type femoral neck fractures received hollow screw internal fixation combined with deep iliac circum flex blood vessels iliac flap graft for treatment. Effects were observed. Results All the 18 cases received follow-up, which showed 16 cases with good postoperative healing, good hip function rehabilitation and no femoral head necrosis. There were 2 cases with late avascular necrosis of femur head, and they received total hip arthroplasty. Conclusion Implement of hollow screw internal fixation combined with deep iliac circum flex blood vessels iliac flap graft is an effective method in treating Pauwels Ⅲ type femoral neck fractures in young adults. It is worth clinical promotion and application.
