摘要
目的 比较后腹腔镜下肾囊肿去顶术联合不同方法预防术后复发的效果。方法 肾囊肿手术患者140例分为三组：在传统后腹腔镜下肾囊肿去顶术（C组54例）基础上,A组40例加用血管夹夹闭囊壁边缘;B组46例加用带蒂脂肪填塞囊肿残腔。观察手术时间、术中出血量和相关并发症,随访术后复发情况。结果 所有病例术后B超、CT检查随访6~24个月,C组7例发现囊肿复发,A、B组无术后囊肿复发。结论 采用后腹腔镜下肾囊肿去顶术联合血管夹夹闭囊壁边缘或带蒂脂肪填塞囊肿残腔能有效预防肾囊肿去顶术后复发。
Objective To compare the efficacy of different technics combined with decortication surgery in preventing recurrence during retroperitoneal laparoscopic surgery for renal cysts. Methods A total of 140 patients with simple renal cyst was randomly assigned into three groups of A （decortication surgery clamping cyst wall edge with vascular clip, 40 cases）, B（decortication surgery＋ plugging pedicled adipose tissues,46 cases） and C（ conventional decortication surgery, 54 cases）. The operative time, blood loss and postoperative complications were observed. The patients were followed up by ultrasonography or CT scanning for 6 to 24 months. Results Postoperative recurrence during follow up was found in 7 cases in group C, which was not seen in groups of A and B. Conclusion decortication surgery combined with clamping cyst wall edge or plugging pedicted adipose tissues is more effective in preventing recurrence during retroperitoneal laparoscopic surgery for renal cysts.
