不同类型细胞因子的表达水平与复发性外阴阴道念珠菌病发病的相关性 被引量:5

Correlation of expression level of different types of cytokines and recurrent vulvovaginal candidiasis

摘要 目的:探究并分析不同类型细胞因子的表达水平与复发性外阴阴道念珠菌病发病的相关性。方法:回顾性分析我院自2012年12月至2014年12月收治的外阴阴道念珠菌病(VVC)患者100例,其中初发患者50例,复发患者50例,按照发病情况分为初发组和复发组。此外,征集正常志愿者50名作为对照组。取各组患者阴道分泌物,ELISA法检测阴道分泌物中IL-12、IL-4、IFN-γ、IL-10、IL-23、HBD-2的表达水平,统计并记录下结果。结果:对照组志愿者IL-4、IL-12、IFN-γ分别为0.35±0.25、0.63±0.31、0.73±0.21,初发组患者IL-4、IL-12、IFN-γ分别为0.33±0.23、1.34±0.38、1.16±0.22,复发组IL-4、IL-12、IFN-γ分别为0.39±0.27、0.74±0.42、1.19±0.23。三组志愿者者IL-4经方差分析差异不明显(F=0.36,P>0.05),而各组IL-12、IFN-γ等具有明显差异(F=15.21,P<0.05;F=83.64,P<0.05),具有统计学意义。各组组间经t检验两两比较,初发组患者IL-12明显高于对照组志愿者和复发组患者(P<0.05),具有统计学意义。初发组患者IFN-γ明显高于对照组志愿者(P<0.05),但复发组与初发组患者之间无明显差异(P>0.05),不具有统计学意义。对照组志愿者IL-10、HBD-2、IL-23分别为(0.526±0.626)ng/L、(8.43±3.46)pg/m L、(1.34±0.39)pg/m L,初发组患者IL-10、HBD-2、IL-23分别为(0.934±0.335)ng/L、(10.23±3.57)pg/m L、(1.13±0.36)pg/m L,复发组患者IL-10、HBD-2、IL-23分别为(1.764±0.619)ng/L、(13.07±3.83)pg/m L、(0.91±0.35)pg/m L,三组志愿者IL-10、HBD-2、IL-23方差分析比较,差异明显(F=9.56,P<0.05;F=7.25,P<0.05;F=6.26,P<0.05),具有统计学意义。初发组和复发组患者IL-10明显高于对照组志愿者(P<0.05),但初发组和复发组患者之间无明显差异(P>0.05)。初发组和复发组患者HBD-2明显高于对照组志愿者(P<0.05),且复发组患者HBD-2明显高于对照组志愿者(P<0.05),具有统计学意义。对照组和初发组患者IL-23无明显差异(P>0.05),但复发组患者IL-23明显高于对照组志愿者(P<0.05),具有统计学意义。结论:复发性外阴阴道念珠菌病患者可能存在一定的免疫缺陷,各类细胞因子的表达异常可能与外阴阴道念珠菌病的发病以及复发有关,但其中具体机制尚未研究清楚,尚待进一步研究。 Objectives:To explore and analyze the correlation of expression level of different types of cytokines and recurrent vulvovaginal candidiasis.Methods:Retrospective analysis was conducted on the 100 cases of vulvovaginal candidiasis treated in our hospital from December 2012 to December 2014.There were 50 primary patients and 50 recurrent patients.In accordance with the incidence,they were divided into primary group and recurrent group.In addition,50 healthy volunteers were chosen as control group.ELISA method was used to test the expression level of vaginal secretions,IL-12,IL-4,IFN-γ,IL-10,IL-23,HBD-2,and the result was recorded.Results:In the control group,the levels of IL-4,IL-12 and IFN-γ were 0.35 ± 0.25,0.63 ± 0.31 and 0.73 ± 0.21.In primary group,the levels of IL-4,IL-12 and IFN-γ were 0.33 ± 0.23,1.34 ± 0.38 and1.16 ± 0.22.In recurrence group,the levels of IL-4,IL-12 and IFN-γ were 0.39 ± 0.27,0.74 ± 0.42 and1.19 ± 0.23.The level of IL-4 in three groups were analyzed by analysis of variance and the difference was not significant( F = 0.36,P 0.05),while the levels of IL-12 and IFN-γ in the three groups has significant difference( F = 15.21,P 0.05;F = 83.64,P 0.05).All patients were tested by t test.The level of IL-12 in primary group was significantly higher than that in control group and recurrent group( P 〈 0.05),with statistically significant difference.The level of IFN-γ in primary group was higher than that in control group( P〈 0.05),but there was no significant difference between recurrence group and primary group( P 0.05).The levels of IL-10,HBD-2 and IL-23 in control group were( 0.526 ± 0.626) ng/L,( 8.43 ± 3.46) pg/m L and( 1.34 ± 0.39) pg /m L.The levels of IL-10,HBD-2 and IL-23 in primary group were( 0.934 ± 0.335) ng/L,( 10.23 ± 3.57)pg/m L and( 1.13 ± 0.36) pg/m L.The levels of IL-10,HBD-2 and IL-23 in recurrent group were( 1.764 ±0.619) ng/L,( 13.07 ± 3.83) pg/m L and( 0.91 ± 0.35) pg/m L.There were significant differences among the three groups( F = 9.56,P 0.05;F = 7.25,P 〈 0.05;F = 6.26,P 0.05).The levels of IL-10 in primary group and recurrent group were significantly higher than that in control group( P 0.05),but no significant difference was found between primary group and recurrent group( P 0.05).The levels of HBD-2 in primary group and recurrent group were significantly higher than that in control group( P 0.05),and the level of HBD-2 in recurrent group was higher than that of control group( P〈 0.05),with statistical significance.The levels of IL-23 in control group and primary group had no significant difference( P〉 0.05),but the levels of IL-23 in recurrentgroup was significantly higher than that in control group( P〈 0.05),with statistical significance.Conclusions:The recurrent vulvovaginal candidiasis patients may have some immunodeficiency.The abnormal expression of various cytokines may be related to the incidence of recurrent vulvovaginal candidiasis as well as the recurrence,but the specific mechanism is not yet clear,which needs further study.
