摘要
目的探讨改良肘后S形切口入路及双侧钢板重建治疗GartlandⅢ型肱骨髁上骨折的临床效果。方法选取2013年1月~2015年1月我院收治的103例肱骨髁上骨折患者,按照入院时间顺序随机分为对照组（53例）与观察组（50例）。对照组接受肘后入路克氏针内固定治疗,观察组接受改良肘后S形切口入路及双侧钢板重建治疗。比较两组的Flynn疗效、骨折愈合时间、骨折解剖学复位率及肘功能Schatzker评分。结果观察组Flynn疗效的优良率为80.0%,高于对照组的52.8%（P〈0.05）。术后3个月,观察组骨折愈合时间短于对照组（P〈0.05）;观察组骨折解剖学复位率为76.0%,高于对照组49.1%（P〈0.05）;观察组肘功能Schatzker评分为（83.12±9.43）分,高于对照组的（71.43±8.59）分（P〈0.05）。结论改良肘后S形切口入路及双侧钢板重建治疗GartlandⅢ型肱骨髁上骨折的效果肯定,可缩短骨折愈合时间,改善术后肘功能,提高患者生活质量,值得临床推广应用。
Objective To explore the clinical effect of modified S shaped incision approach combined with bilateral plate reconstruction in the treatment of Gartland type Ⅲ supracondylar fracture of humerus. Methods 103 patients with supracondylar fracture of humerus admitted into our hospital from January 2013 to January 2015 were selected.According to the time of admission,they were randomly divided into the control group（53 cases） and the observation group（50cases）.The control group was received posterior approach of elbow kirschner internal fixation,the observation group was received modified S shaped incision approach and bilateral plate plate reconstruction.Flynn effect,the healing time of fracture,fracture anatomical reduction rate and Schatzker score in two groups was compared. Results The effective rate in observation group was 80.0%,which was higher than 52.8% of control group（P 0.05）.3 months after operation,the healing time of fracture in observation group was shorter than the control group（P〈0.05）.The anatomic reduction rate in observation group was 76.0%,which was higher than 49.1% of control group（P〈0.05）.The function of Schatzker score in observation group was（83.12±9.43） points,which was higher than（71.43±8.59） points of control group（P〈0.05）. Conclusion Modified S shaped incision approach and bilateral plate reconstruction in the treatment of Gartland type Ⅲsupracondylar fracture of humerus is a good result,can shorten the time of fracture healing,improve postoperative elbow function,improve patients′ quality of life.It is worth extending in clinical practice.
