摘要
目的探讨甜味剂联合非营养性吸吮对早产儿静脉穿刺所致疼痛的影响。方法选择在NICU住院早产儿200例随机分为常规组、非营养性吸吮组、葡萄糖+非营养性吸吮组、蔗糖+非营养性吸吮组各50例,在静脉穿刺前2 min给予安慰奶嘴和经奶嘴旁注入0.5 ml相应浓度的甜味剂,检测记录穿刺后各组早产儿心率、血氧饱和度变化及疼痛评分指标。结果静脉穿刺后1 min各组早产儿疼痛评分、心率、血氧饱和度分别为:常规组为(13.529±1.58 58)、(175.17±10.749 83)、(98.0588±2.192 9),非营养性吸吮组为(11.611±2.592 7)、(159.88±11.457 22)、(98.384 6±1.551 18),葡萄糖+非营养性吸吮组为(10.308±1.517 1)、(157.61±12.180 69)、(98.625±1.302 47),蔗糖+非营养性吸吮组为(4.375±2.263 8)、(152.27±12.173 93)、(99.055 6±0.802 37);静脉穿刺后5 min各组早产儿分别为:常规组为(11.411 8±1.277 64)、(148.82±20.439 65)、(98.875±0.991 03),非营养性吸吮组为(10.576 9±1.527 69)、(147±11.892 37)、(98.937 5±0.928 71),葡萄糖+非营养性吸吮组为(9.333 3±2.890 15)、(142.92±10.268 1)、(99.692 3±0.470 68),蔗糖+非营养性吸吮组为(3.5±2.563 48)、(133±10)、(99.71±0.891 58)。结论静脉穿刺后1、5 min非营养性吸吮、葡萄糖+非营养性吸吮、蔗糖+非营养性吸吮均有缓解静脉穿刺致早产儿疼痛的效果,且蔗糖+非营养性吸吮的镇痛效果优于非营养吸吮和葡萄糖+非营养性吸吮。
Objective To explore the effect of edulcorant combined with non-nutritiye sucking on pain induced by venipuncture in pre- mature infants. Methods Two hundred hospitalized premature infants were selected from NICU and randomly divided into routine therapy group, non-nutritive sucking group, glucose combined with non-nutritive sucking group, sucrose combined with non-nutritive sucking group, 50 infants in each group. At 2 minutes before venipuncture, 0. 5 ml corresponding concentration of edulcorant was given to the in- rants. After five minutes of venipuncture, pain scores, heart rates, and blood oxygen saturation values were (11. 411 8± 1. 277 64), ( 148.82±20. 439 65), (98. 875±0. 991 03) in routine therapy group, ( 10. 576 9±1. 527 69), ( 147±11. 892 37), (98. 937 5±0. 928 71 ) in non-nutritive sucking group, (9. 333 3±2. 890 15 ) , ( 142. 92± 10. 268 1 ) , (99. 692 3±0. 470 68 ) in glucose combined with non- nutritive sucking group, ( 3.5 ±2. 563 48 ), ( 133-+ 10 ), ( 99.71 ±0. 891 58 ) in sucrose combined with non- nutritive sucking group. Con- clusion After one and five minutes of venipuncture, non-nutritive sucking, glucose combined with non-nutritive sucking, and sucrose combined with non-nutritive sucking can effectively relieve the pain induced by venipuncture in premature infants, and the analgesic effect of sucrose combined with non-nutritive sucking is better than those of the other two methods.
