摘要
目的研究唑来膦酸钠对绝经后2型糖尿病合并骨质疏松患者腰椎骨密度及骨代谢的影响。方法选择2012年2月至2014年4月收治的绝经后2型糖尿病合并骨质疏松患者53例，均根据血糖高低服用降糖药物或注射胰岛素调节血糖浓度，控制血糖值基本正常，同时给予唑来膦酸钠。结果治疗后患者空腹血糖水平显著下降（t=24．084，P〈0．05），糖化血红蛋白水平显著降低（t=15．148，P〈0．05）。治疗后患者三酰甘油和低密度脂蛋白水平显著降低（t=2．387—2．496，P〈0．05）；治疗前后两组患者总胆固醇和高密度脂蛋白水平差异无统计学意义（t=1．279—1．629，P〉0．05）。治疗后患者甲状旁腺素和25（0H）VD均显著升高（f：2．309-2．005，P〈0．05）；治疗前后，患者骨特异性碱性磷酸酶水平差异无统计学意义（t=0．309—1．521，〉0．05）；治疗后患者L2,L3和L2-4骨密度均显著增加（t=2．236～3．247，P〈0．05）；治疗后患者L4骨密度虽有所增加，但差异无统计学意义（t=1．428，P〉0．05）。结论唑来膦酸钠能显著增加绝经后2型糖尿病合并骨质疏松患者腰椎骨密度．改善骨代谢指标。
Objective To study the effect of sodium phosphate on bone mineral density and bone metabolism in postmenopausal patients with type 2 diabetes mellitus.Methods 53 diabetic patients cured in our hospitalfrom February 2012 to April 2014 with osteoporosis in patients as research object. All the selected subjects were treated according to the level of blood glucose or glucose lowering drugs or injection of insulin to regulate blood glucose concentration.Results The fasting blood glucose levels were significantly decreased（t=24.084,P〈 0.05）,and the level of glycosylated hemoglobin（t=15.148,P〈 0.05） was significantly lower af- ter the treatment. After treatment, the triglyceride and low density lipoprotein cholesterol levels decreased significantly （t=2.387- 249.6 ,P 〈 O.O5 ）.Before and after treatment , the difference of total cholesterol and high density lipoprotein level were no statisti- cal significance （t=l.279-1.629,P〉O.05）. After treatment, patients with parathyroid hormone and 25 （OH）VD were significantly increased （t=2.309-2.005,P〈 O.05）.Before and after treatment , the difference of bone specific alkaline phosphatase levels were no statistical significance（t=0.309-1.521,P〉 0.05）; After treatment, the patients of L2,L3 and L2-4 BMD increased significantly （t= 2.236-324.7,P 〈 0.05）.Conclusion Sodium phosphate can significantly increase the bone mineral density and bone metabolism in postmenopausal patients with type 2 diabetes mellitus combined with osteoporosis.
