外周血OPN、CK-19及MUC1表达水平在乳腺癌中的临床意义 被引量:11

摘要 目的 探讨外周血骨桥蛋白(OPN)、细胞角蛋白19(CK-19)、黏蛋白1(MUC1)的表达水平在乳腺癌中临床意义。方法 选择2013年2月至2015年2月攀枝花市中心医院收治的50例乳腺癌患者作为观察组,同期选择30例乳腺良性肿瘤的患者作为对照组,20例健康体检者为健康组,应用反转录聚合酶链反应(RT-PCR)技术测定患者外周血中OPN、CK-19及MUC1表达情况,以分析三者在乳腺癌诊断中的作用。结果 观察组OPN、CK-19及MUC1阳性表达率分别为62.0%(31/50)、30.0%(15/50)和48.0%(24/50),而对照组仅2例OPN表达阳性,其余均为阴性,健康组中三者均为阴性。观察组OPN、CK-19及MUC1阳性表达率高于对照组和健康组,差异有统计学意义(P〈0.05)。Ⅲ-Ⅳ期患者OPN、CK-19及MUC1阳性表达率高于Ⅰ-Ⅱ期[86.4%(19/22)比42.9%(12/28),54.5%(12/22)比7.1%(2/28),68.2%(15/22)比32.1%(9/28)],差异有统计学意义(P〈0.05)。有淋巴结转移患者OPN、CK-19及MUC1阳性表达率高于无淋巴结转移者[81.8%(18/22)比42.9%(12/28),45.5%(10/22)比14.3%(4/28),54.5%(12/22)比39.3%(11/28)],差异有统计学意义(P〈0.05)。结论 OPN、CK-19及MUC1表达水平在乳腺癌患者中明显上调,并且在Ⅲ期和Ⅳ期患者或淋巴结转移患者中的阳性表达率更高。提示其对乳腺癌微转移有一定预测作用。 Objective To explore the clinical significance of peripheral blood osteopontin ( OPN), cytokexatin protein 19 (CK-19) and mucin 1 ( MUC1 ) expression levels in breast cancer. Methods From Feb. 2013 to Feb. 2015,50 breast cancer patients in Panzhihua City Central Hospital were chosen as the research objects, and 30 benign breast tumor patients during the same period were included as contrel group, 20 healthy people for health checkup were included.as healthy group, and reverse transcription pdymerase chain reaction (RT-PCR) technique was used to test'the peripheral blood OPN, CK-19 and MUC1 expression in order to analyze the role of the three in the diagnosis of breast cancer. Results OPN, CK-19 and MUC1 expression rates of the observation group were 62.0% (31/50),30. 0% (15/50) and 48.0% (24/50), while in the control group, only 2 cases of OPN expression was positive, the rest were negative, in the healthy group all three indexes were negative. OPN, CK-19 and MUC1 expression rate of the observation group were higher than the control group and healthy group, the differences were statistically significant ( P 〈 0. 05 ). OPN,CK-19 and MUC1 expression of stage III-IV were higher than stage I- II [ 86. 4% (19/22) vs 42. 9% (12/28) ,54. 5% (12/22) vs 7. 1% (2/28) ,68.2% (15/22) vs 32. 1% (9/28) ] ,the differences were statistically significant( P 〈 0. 05 ). OPN, CK-19 and MUC1 positive expression rate of patients with lymph node metastasis were higher than patients without lymph'node metastasis [ 81.8% (18/22) vs 42. 9% (12/28) ,45.5 % ( 10/22 ) vs 14. 3% ( 4/28 ) ,54. 5% (12/22) vs 39.3 % ( 11/28 ) ], the differences were statistically significant(P 〈 0. 05 ). Conclusion The levels of OPN, CK-19 and MUC1 are significantly up-regnlated in breast cancer patients,and the expression levels in patients of stage III and IV or patients with lymph node metastasis are higher, which indicates a certain predictive effect on the micro metastasis of breast cancer.
