摘要
目的：利用Meta分析观察包皮环切术治疗早泄的疗效。方法检索Pubmed、Embase、Cochrane图书馆，VIP数据库，CNKI数据库，SinoMed系统，万方期刊数据库，纳入包皮环切术治疗早泄的随机对照试验（RCT），并进行方法学质量评价，采用RevMan 5.2软件进行统计学分析。结果8篇RCT文献纳入研究，累计739例病例。Meta分析结果显示：与空白对照组比较性生活满意度无改善[异质性：Tau2=0.21； Chi2=5.29, df=1（P=0.02）；I2=81%；整体效果检验：Z=1.05（P=0.30）]，[RR=1.46，95%CI（0.72，2.98）, P=0.30]，与其他治疗方案比较早泄改善[Heterogeneity： Tau2=0.10； Chi2=20.32,df=3（P=0.0001）； I2=85%； Test for overall effect： Z=0.44（P=0.66）],[RR=1.08,95%CI（0.77，1.51）,P=0.66]，射精潜伏期延长[Heterogeneity： Tau2=3.27； Chi2=105.08, df=2（P＜0.00001）； I2=98%；Test for overall effect： Z=0.33（P=0.74）],[SMD=0.35,95%CI（-1.72，2.41）,P=0.74]。结论包皮环切术对于早泄有改善作用，但其确切的治疗作用还需要进一步开展高质量、大样本的随机对照试验予以证实。
Objective To evaluate the efficacy of circumcision for premature ejaculation.Methods The randomized controlled trials （RCTs） were collected from database, such as Pubmed, Embase, the Cochrane library, VIP, CNKI, SinoMed, Wanfang. After the necessary methodological quality evaluation of the selected studies, the statistics analysis was conducted by the software RevMan 5.2.Results Eight studies involving 739 patients were included. In comparision with the blank control group, the results showed an increase in sexual satisfaction [Heterogeneity：Tau2=0.21;Chi2=5.29, df=1（P=0.02）;I2=81%; Test for overall effect： Z=1.05（P=0.30）], [RR=1.46, 95%CI（0.72, 2.98）,P=0.30]. In comparision with the other treatment groups, the results showed a significant decrease in that premature ejaculation[Heterogeneity： Tau2=0.10;Chi2=20.32, df=3（P=0.0001）; I2=85%; Test for overall effect：Z=0.44（P=0.66）], [RR=1.08, 95%CI（0.77, 1.51）,P=0.66], prolonging in the intravaginal ejaculation latency times [Heterogeneity： Tau2=3.27; Chi2=105.08, df=2（P〈0.00001）; I2=98%;Test for overall effect： Z=0.33（P=0.74）], [SMD=0.35, 95%CI（-1.72, 2.41）,P=0.74].Conclusion Circumcision may be an effective treatment for premature ejaculation, but due to the limitations of included RCTs, high quality and large sample size clinical RCTs are suggested in future.
