摘要 目的探讨3.0T磁共振T_2-mapping成像对早期膝关节软骨损伤的诊断价值。方法收集我院临床根据美国风湿病学会膝关节骨关节炎(OA)诊断标准诊断为OA的25例患者,为实验组,分为轻度OA组和重度OA组;另筛选到20例健康志愿者共20个膝关节为对照组。采用Siemens 3.0T MRI常规扫描及T_2-mapping序列进行膝关节及关节软骨成像;将map扫描所得图像数据传送至工作站,利用Syngo via后处理软件,直接获得关节软骨的T_2-mapping伪彩图像,并进行T_2值数据测量,分别观察和测量胫骨内侧平台、外侧平台、股骨内侧髁、外侧髁4处软骨的T_2值,行统计学分析。结果正常组、轻度OA组、重度OA组关节软骨的平均T_2值分别为(37.2±1.6)ms、(48.6±0.2)m、(49.3±0.8)ms。轻度OA组、重度OA组膝关节软骨各部分软骨T_2值较正常组有明显增高,P<0.05,差异有统计学意义。轻度OA组、重度OA组各区域软骨T_2值比较无显著性差异,P>0.05,两组差别无统计学意义。结论 T_2-mapping成像能早期发现未发生形态学改变的OA患者软骨组织成分变化,是评价膝关节软骨早期损伤的重要诊断手段。 Objective To explore the diagnostic value with T2-mapping of 3.0T MRI in the early knee cartilage injury. Methods T2 relaxation time of knee cartilage was evaluated with Siemens 3.0T MR in 20 helathy knee joints(age range :22-43 years,mean age 38±3.2 years) and 25 knee joints of osteoarthritis(OA)(age range:35-59 years,mean age 45±4.6 years). The T2 values of medial femur、lateral femur、medial tibia、lateral tibia cartilage were measured for subsequent satistical analysis. Result The average T2 relaxation time for knee cartilage of normal group,serere OA group,and severity OA group were(37.2±1.6)ms,(48.6±0.2)ms and(49.3±0.8)ms. There was a significant difference in T2 value between case group and control group(P〈0.05),but no significance was found be-tween the mild and severe OA group(P〉0.05). Conclusion T2-mapping can early indiciates changes in compositions of the cartilage in patients with OA who had no signaificant morphological abnormality,and could be considered as an important sensitive method to detect early cartilage degeneration.
