血气分析与慢性阻塞性肺疾病急性加重期肺功能的相关性 被引量:1

摘要 目的探讨血气分析中各指标与慢性阻塞性肺疾病急性加重期(AECOPD)肺功能变化的相关性,为临床诊治提高依据。方法收集我院呼吸科就诊住院的AECOPD患者50例。采用肺功能检测仪监测并计算第1秒用力呼气容积占预计值的百分比(FEV_1%prep)及第1秒用力呼气容积/用力肺活量比(FEV_1/FVC);血气分析仪监测动脉血pH、动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)。并进行FEV_1%prep和FEV_1/FVC与pH、PaO_2及PaCO_2相关性分析。结果 AECOPD患者FEV_1%prep和FEV_1/FVC与pH无相关性(r=0.125,0.136,P>0.05);与PaO_2呈正相关(r=0.654,0.744,P<0.05);与PaCO_2呈负相关(r=-0.225,-0.134,P<0.05)。结论血气分析指标PaO_2和PaCO_2与慢性阻塞性肺疾病急性加重期肺功能密切相关,可以作为诊断和评估患者预后的指标。 Objective To investigate the correlation of blood gas and pulmonary function changes in (AECOPD), for clinical diagnosis and treatment. Methods 50 patients with AECOPD were collected in our hospital, the percent of expected forced expiratory volume in 1 second (FEV1% prep) and forced expiratory volume in 1 second/forced vital capacity (FEV1/FVC) were monitored and calculated by lung fimction detector; the correlation analysis was done between (FEV1% prep) and FEV]FVC with pH, PaO2 and PaCO2. Results the patients with AECOPD (FEV~% prep) and FEV]FVC had no correlation with pH (r=0.125, 0.136, P〉0.05); had positively correlated with PaO2 (r=0.654, 0.744, P〈0.05); And PaCO2 show negative correlation (r=-0.225, -0.134, P〈0.05). Conclusion The index of blood gas analysis PaO2 and PaCO2 in chronic obstructive pulmonary disease with acute aggravating period had closely related to the pulmonary function, can be used as a diagnostic index and evaluate the prognosis of patients.
