摘要
目的探讨前纵隔胸腺瘤和淋巴瘤的^(18)F-FDGPET/CT显像特征及鉴别诊断。方法分析经病理证实的14例胸腺瘤和14例淋巴瘤的临床资料及^(18)F-FDGPET/CT显像资料,观察不同肿瘤^(18)F-FDG PET/CT显像特征,测量病灶最大径及最大标准摄取值(SUVmax)。结果 14例胸腺瘤中10例发生于40岁以上,3例合并重症肌无力,肿块多局限于前纵隔且偏向一侧,密度不均匀,周围血管结构受压移位。14例前纵隔淋巴瘤中12例发生于40岁以下,8例伴有发热,肿块多为多结节融合成团块状,双侧性,密度均匀,并侵犯中后纵隔,包绕周围血管结构。淋巴瘤的SUVmax(13.8±4.1)高于胸腺瘤(8.7±4.4),差异有统计学意义(Z=-2.736,P=0.006),而2组间最大径差异无统计学意义(Z=-0.322,P=0.748)。结论前纵隔胸腺瘤和淋巴瘤各自具有一定的^(18)F-FDGPET/CT显像特征,结合临床,能够作出正确诊断。
Objective To investigate the manifestations of ^18F-FDG PET / CT scanning and differential diagnosis of thymoma and lymphoma in the anterior mediastinum,in order to improve the correct diagnostic rate of them. Methods ^18F-FDG PET / CT images and clinical data of 14 cases of thymoma and 14 of lymphoma confirmed by pathology were retrospectively analyzed. The manifestations of ^18F-FDG PET / CT were observed and the maximum diameter and uptake of ^18F-FDG of tumors were measured.Results In 14 patients of thymoma,10 cases occurred over of 40 years and 3 cases in combination with myasthenia gravis.18 FFDG PET / CT of thymoma showed irregular tumors with heterogeneous density located in a side of anterior mediastinal and pushed the neighboring vessel structures. In 14 patients of lymphoma,12 cases occurred under of 40 years and 8 cases in combination with fever. ^18F-FDG PET / CT of lymphoma showed crumb tumors mixed by several nodes with homogeneous density at the both side of anterior mediastinum,and invaded the middle and posterior mediastinum. The neighboring vessel structures were enveloped by lymphoma. The SUVmax of lymphoma( 13. 8 ± 4. 1) was higher than thymoma( 8. 7 ± 4. 4) with statistical significance( Z =-2. 736,P = 0. 006). There were no differences of tumor maximum diameter between the two groups( Z =-0. 322,P =0. 748). Conclusion The anterior mediastinal thymoma and lymphoma have certain characteristics of ^18F-FDG PET / CT imaging respectively. In combination with the clinical informations,the diagnosis can be made correctly.
