摘要
目的探讨踝关节骨折合并下胫腓联合韧带损伤患者应用手术治疗的效果。方法选择2012年3月至2014年3月期间来我院骨科就诊的78例踝关节骨折合并下胫腓联合韧带损伤的患者为研究对象,根据患者及家属意愿分为两组。39例观察组患者采取手术治疗,于术中进行内外踝固定、下胫腓联合固定治疗;39例对照组患者采取非内固定手术治疗方案。比较两组患者的临床治疗效果。结果观察组的手术优良率为94.87%,显著高于对照组的79.49%,差异有统计学意义(P<0.05);观察组的二次手术发生率为5.13%,显著低于对照组的20.51%,差异有统计学意义(P<0.05)。结论对于踝关节骨折合并下胫腓联合韧带损伤患者的治疗,在固定内外踝的同时应用螺钉固定下胫腓联合,有利于提高骨折愈合的优良率,降低二次手术的发生率,值得临床应用。
Objective To explore the clinical effect of operative treatment for patients with ankle fractures complicated with syndesmosis ligament injury. Methods A total of 78 cases of patients with ankle fractures complicated with syndesmosis ligament injury admitted to our hospital from March 2012 to March 2014 were selected and divided into two groups according to patients＇ and dependents＇ willing. 39 cases of observation group received internal and external malleolus fixation, and syndesmosis fixation during operation, while the control group received operation without internal fixation. The clinical effects of two groups were observed and compared. Results The excellence and good rate of observation group was 94.87%, significantly higher than 79.49% of control group, with statistical difference （P 〈0.05）. The rate of second operation of observation group was 5.13%, significantly lower than 20.15% of control group, with statistical difference （P〈0.05）. Conclusions For patients with ankle fractures complicated with syndesmosis ligament injury, the fixation of internal and external malleolus combined with syndesmosis fixation by screws can effectively improve the excellence and good rate of fracture healing, and reduce the rate of second operation, which is worthy of clinical application.
