摘要
目的：探讨某院针对Ⅰ类切口围手术期预防使用抗菌药物的持续干预措施对该院等级医院评审结果的影响。方法：利用合理用药监测系统选取该院2015年4-12月Ⅰ类切口手术患者出院病例,每轮随机抽样1 500例,根据干预措施实施的3个循环过程分为3组（第1轮干预组、第2轮干预组、第3轮干预组）,对围手术期预防性使用抗菌药物的2 470份病历进行回顾性调查,分析持续干预过程中Ⅰ类切口围手术期抗菌药物合理使用的持续改进情况。结果：在数轮干预过程中,该院Ⅰ类切口手术围手术期抗菌药物预防使用率逐渐降低,用药合理率逐渐提高,且在选择抗菌药物类型方面也不断改进,此次干预措施促进了该院等级医院评审评分结果的提升。结论：该院针对Ⅰ类切口围手术期抗菌药物预防使用的持续干预措施有效地促进了等级医院评审相关条款的达标,对医院管理的质量的提升具有积极影响。
Objective：To describe the impact of interventions in prophylactic antibiotic use, particularly in Type Ⅰ incision operations, on improving result of grade accreditation of medical institutions. Methods： Patients who received Type Ⅰ incision operations from April 2015 to December 2015 were enrolled via PASS software. The patients were assigned into three gruops ： the 1st intervention cycle, 2nd intervention cycle and 3rd intervention cycle （1 500 patients selected randomly per cycle）, except the cases which were erroneous judged by PASS and were related to infections before operations. The rationality of antibitic use in Type Ⅰ incision operations throughout the 2 470 patient records was retrospectively analyzed and evaluated. Results： The study showed that the rate of antibiotie prophylaxis in Type Ⅰ incision operations was decreased, and the percentage of appropriate prescriptions was increased through the interventions, prescriptions adhering to the criteria in the choice of antibiotics was also improved through the interventions. Outcomes from the interventions contributed to enhancement of the hospital＇ s performance in grade accreditation of medical institutions. Conclusion： The study illustrated that the interventions in prophylatic use in Type Ⅰ incision operations contributed to improving the result of grade accreditation of medical institutions. It had a positive influence to the promotion of the quality of the hospital management.
