血清降钙素原与超敏C-反应蛋白在新生儿感染性疾病中的表达及其临床评价 被引量:52

Applications of procalcitonin and hypersensitive C-reactive protein expression in neonatal infectious diseases

摘要 目的探讨血清降钙素原(PCT)与超敏C-反应蛋白(hs-CRP)在新生儿感染性疾病中的表达及其临床价值,以指导临床早期治疗。方法选取2013年1月-2014年12月医院收治的感染性疾病新生儿86例作为感染组,根据感染病原体将其分为细菌感染组52例与非细菌感染组34例;另选取医院同期收治的非感染性疾病新生儿80例作为对照组,分别采用电化学发光法与免疫乳胶凝集法对所有研究对象的血清PCT与hs-CRP水平进行检测,并对检测结果进行比较分析。结果与对照组比较,细菌与非细菌感染组新生儿PCT及hs-CRP水平均明显升高,差异有统计学意义(P<0.05);与非细菌感染组比较,细菌感染组新生儿PCT及hs-CRP水平均明显升高,差异有统计学意义(P<0.05);PCT检测特异度高于hs-CRP,差异有统计学意义(P<0.05);而PCT与hs-CRP联合检测的敏感度与特异度较PCT、hs-CRP单独检测明显提高,差异有统计学意义(P<0.05)。结论 PCT、hs-CRP可以作为诊断新生儿感染性疾病的有效指标,且PCT与hs-CRP联合检测的特异度、敏感度更高,两者联合检测具有更高的临床价值。 OBJECTIVE To investigate serum procalcitonin(PCT)and hypersensitive C-reactive protein(hs-CRP)expression in neonatal infectious diseases and its clinical value,so as to guide early clinical treatment.METHODS A total of 86 cases of newborn with infectious diseases in our hospital from Jan.2013 to Dec.2014 were selected as infection group,and divided into bacterial infection group(n=52)and non-bacterial infection group(n=34)according to the pathogens.Another 80 cases of newborn with no infectious disease during the same period were selected as control.Electrical chemiluminescence immunoassay and immune latex agglutination reaction were used for detection of serum calcitonin original(PCT)and allergic C-reactive protein(hs-CRP),and the test results were compared.RESULTS Compared with control group,the PCT and hs-CRP levels of bacterial infection group and non-bacterial infection group were significantly increased(P〈0.05).Compared with non-bacterial infection group,the PCT and hs-CRP levels of bacterial infection group were significantly increased(P〈0.05).The specific degree of PCT was significantly higher than that of hs-CRP(P〈0.05).And the sensitivity and specific degree of joint detection of PCT and hs-CRP were significantly improved than detection alone(P〈0.05).CONCLUSION PCT and hs-CRP can be used as effective indexes of the diagnosis of neonatal infectious diseases,and the joint detection of PCT and hs-CRP has higher sensitivity and specific degree,which has higher clinical value.
