Applicative Value of Dynamic Enhanced MRI Combined with CT in Diagnosis and Staging of Endometrial Carcinoma

摘要 目的评价磁共振成像(MRI)动态增强结合CT在子宫内膜癌诊断及分期中的应用价值。方法选择2013年1月至2014年12月在重庆市中医院就诊的子宫内膜癌疑似患者210例,分别采用MRI、CT扫描和术后病理学检查进行诊断,计算诊断符合率和病理学分期准确率。结果 MRI动态增强结合CT诊断准确度[受试者工作特征曲线下面积(AUC)=0.954]高于单独CT扫描(AUC=0.864)和单独MRI(AUC=0.921);MRI动态增强结合CT、单独CT扫描、单独MRI与病理学分期的一致性Kappa值分别为0.883、0.670和0.743,MRI动态增强结合CT一致性明显高于单独CT扫描和MRI扫描。结论 MRI动态增强结合CT能准确诊断子宫内膜癌,对肿瘤术前分期及选择治疗手段有重要意义。 Objective To assess the applicative value of dynamic enhanced magnetic resonance imaging (MR/) and CT in the diagnosis and staging of endometfial carcinoma. Methods Total of 210 patients with suspected endometrial carcinoma in Chongqing Traditional Chinese Medicine Hospital from Jan. 2013 to Dec. 2014 were included in the study, they received diagnosis by the methods of MRI, CT and postoperative pathology respectively, and the coincidence rate of diagnosis and pathology staging accuracy were calculated. Results The diagnostic accuracy of dynamic enhanced MR/combined with CT [ the area under the curve (AUC) = 0. 954 ] was higher than CT scan( AUC = 0. 864)and MR/scan( AUC = 0. 921 ) alone; the consistency of dynamic enhanced MRI combined with CT, CT scan and MR/ scan with pathology staging were 0. 883,0. 670 and 0. 743 respectively, and the eorisisteney of the dynamic enhanced MR/combined with CT was significantly higher than CT and MR/ scans. Conclusion Dynamic enhanced MR/ combined with CT can accurately diagnose endometrial careinoma,and it has great significance for the preoperative staging and choice of treatment.
