摘要
目的探讨改良Praks切口在斜视手术中的应用及对患者泪膜功能的影响。方法 250例斜视患者随机平均分为两组：对照组125例,采用角膜缘梯形结膜瓣切口;观察组125例采用改良Praks切口,比较两组患者治疗疗效及患者泪膜功能变化。结果观察组治愈125例,治愈率为100%,显著高于对照组治愈率（105例,87.2%）,差异具有统计学意义（χ^2=6.49,P=0.00）;与术前相比,两组患者干眼症状评分均升高后再恢复至术前水平,但对照组升高程度高于观察组（P〈0.05）;观察组术后7 d和10 d的干眼症状评分显著低于对照组（P〈0.05）;术前,两组患者泪膜功能参数（Slt、BUT和CFS值）无统计学差异（P〉0.05）;手术后,两组患者术后7 d时的泪膜功能参数（Slt和CFS值）均显著增加（P〈0.05）,BUT显著降低（P〈0.05）;但观察组术后7 d时的Slt和CFS值低于对照组（P〈0.05）,BUT值高于对照组,差异显著（P〈0.05）;两组患者术后28 d的泪膜功能参数恢复至术前水平,差异无统计学意义（P〈0.05）;观察组睑裂外观良好、术后炎症反应小、干眼情况少和患者满意率均显著高于对照组,差异显著（P〈0.05）。结论改良Parks切口斜视手术能够有效矫正患者斜视症状,临床疗效好,且对患者泪膜功能影响小,最大范围地达到了患者治疗和美容的共同效果,值得临床推广使用。
Objective To investigate the modified application Praks incision and its effect on tear film functions in strabismus surgery patients. Methods Totally 250 strabismic patients were randomly divided into two groups： 125 patients received limbal conjunctival flap incision trapezoid（ control group） and another 125 patients received improved Praks incision. Treatment efficacy and tear function changes were compared. Results All patients received Praks incision were cured and the cure rate（ 100%） was significantly higher than in control group（ 87. 5%; P〈0. 01）. Ratings of dry eye symptom scores were improved in both groups but significantly better at 7 days and 10 days in patients who received Praks incision（ P〈0. 05）. There was no difference in baseline level of tear film tests,e. g.,Slt,BUT and CFS values between the two groups. Seven days after the surgery,all these parameters significantly improved in both groups（ both P〈0. 05）but the improvements were significantly better in patients received Praks incision（ P〈0. 05）. At 28 days,there was no difference in tear film function tests between the two groups. Overall,the surgical outcomes,such as inflammation,dry eye symptoms and patient satisfaction,were significantly better when patients received Praks incision. Conclusions Modified Parks incision surgery can effectively correct strabismus strabismus symptoms,minimally affect the tear film function,and result in maximally clinical outcomes.
