摘要
目的:评价皮下负压引流对妇科恶性肿瘤患者切口引流液中PDGF、TGF-β含量的影响。方法:接受手术治疗的妇科恶性肿瘤患者80例,分为无负压组(30例)、低负压组(30例)及高负压组(20例),三组腹壁切口均给予自然对合处理,并分别实施不同强度的腹壁负压引流,监测三组切口愈合状况及引流液中PDGF及TGF-β含量。结果:无负压组腹壁切口脂肪液化发生率高于高负压组及低负压组(P<0.05),切口甲级愈合率明显低于低负压组及高负压组(P<0.05),低负压组、高负压组切口导管留置时间及术后第3天切口引流量显著少于无负压组(P<0.05)。术后第3天低负压组、高负压组腹壁切口引流液中PDGF和TGF-β含量高于无负压组(P<0.05),而高负压组PDGF含量高于低负压组(P<0.05)。结论:高负压引流对妇科恶性肿瘤手术切口愈合过程具有一定的促进作用。
Aim:To evaluate the effects of abdominal wall incision natural involution and subcutaneous negative pres -sure drainage combination on PDGF and TGF-βlevels in abdominal wall incision drainage fluid in gynecologic malignant tumor patients .Methods:A total of 80 patients with gynecological malignant tumor received surgical treatment were ran -domly assigned to non-negative pressure group(30 cases), low negative pressure group(30 cases) and high negative pres-sure group(20 cases).Three groups were treated with incision of abdominal wall incision natural involution , and implement the different strength of abdominal wall drainage , and incision healing and PDGF and TGF-βlevels in drainage fluid were monitored .Results:The incidence of fat liquefaction in the non-negative pressure group was significantly higher than those in the high negative pressure group and low negative pressure group (P〈0.05),and the wound healing rate was significant-ly lower than those of the low negative pressure group and high negative pressure group (P〈0.05).The catheter retaining time and drainage volume during 3rd days after operation of low negative pressure group and high negative pressure group were significantly lower than those in the non-negative pressure group (P〈0.05).The PDGF and TGF-βlevels in low negative pressure group and high negative pressure group were significantly higher than those of non -negative pressure group at the 3rd day after operation(P〈0.05), and level in high negative pressure group was higher than that of low negative pressure group(P〈0.05).Conclusion:The combined application of abdominal incision natural involution and subcutane-ous negative pressure drainage can promote the wound healing process of patients with gynecologic malignant tumors .
