摘要
点评中国和美国急性缺血性脑卒中诊疗指南的差异,肯定指南的意义和影响,也指出其存在缺陷与不足。本文提出国内指南的编写目的和使用者需要延扩,指南制定者和内容涵盖应进行完善,对脑卒中公众教育、卒中救治体系和质量改进应进行补充,针对院前管理需要细化并强调院前急救体系的作用,在卒中症状体征识别方面应全面,诊断流程部分则需要优化,提出静脉溶栓应该规范和进行质量监管,指出脑卒中病因分型选择存在滞后。以此启示我国脑血管病医疗实践与国外的差距,同时提出部分改进意见,为下一版指南的修订提供参考和依据。
This thesis comments on the differences between Chinese and American guidelines for the diagnosis and treatment of acute ischemic stroke,affirms the significance and influence of the guidelines,and also points out the defects and deficiencies.In the Chinese guidelines,we propose the purpose of writing and users need to extend.Besides,guide makers and content coverage should be improved; What is more,public education,stroke treatment system and quality improvement of cerebral apoplexy should be supplemented.In view of the fact that the pre-hospital management needs to refine and emphasize the role of pre hospital emergency care system,the identification of signs and symptoms of stroke should be comprehensive,and part of the diagnostic process needs to be optimized.Moreover,it presents the view that venous thrombolysis should be standardized and quality should be supervised and that there is a lag in the etiology of stroke,in order to enlighten the gap between the medical practice of cerebrovascular disease in China and the foreign countries Some suggestions are put forward to provide reference and basis for the revision of the next edition.
