摘要
目的:研究低剂量甲基强的松龙对预防下肢长骨骨折患者创伤后肺脂肪栓塞的作用。方法:选择我院2014年1月~2015年6月骨科收治的胫骨及股骨骨折患者96例,采用随机数字表法分为三组,每组32例,分别给予低剂量甲基强的松龙、大剂量甲基强的松龙及生理盐水,三组患者年龄、性别、骨折部位及严重程度均进行均衡性对比,观察三组肺脂肪栓塞发生情况。结果:治疗3d、5d、7d低剂量甲基强的松龙组、大剂量甲基强的松龙组FES评分较治疗前无明显变化(P〉0.05)。对照组治疗3d、5d、7d FES评分明显升高(P〈0.05),与低剂量甲基强的松龙组、大剂量甲基强的松龙组比较差异有统计学意义(P〈0.05)。三组共发生FES5例,其中低剂量甲基强的松龙组1例(3.13%),大剂量甲基强的松龙组0例(0.0%),对照组4例(12.5%),对照组FES发生率明显高于低剂量甲基强的松龙组、大剂量甲基强的松龙组(P〈0.05)。低剂量甲基强的松龙组、大剂量甲基强的松龙组FES发生率比较差异无统计学意义(P〉0.05)。结论:低剂量甲基强的松龙可预防下肢长骨骨折患者创伤后肺脂肪栓塞。
Objective:to study the low doses of methyl prednisolone for prevention of lower limbs long bone fracture patients with posttraumatic pulmonary fat embolism. Methods:from January 2014 ~ June 2015 orthopaedic treated 96 cases of tibial and femoral fracture patient,using the digital meter random method was divided into three groups,each group of 32 cases,were given low doses of methyl prednisolone,large dose of methyl prednisolone and physiological saline,three groups of patients age,sex,fracture and severity are balance comparison, observe three groups of pulmonary fat embolism is happening. Results: 3d, 5d, 7d low doses of methyl prednisolone FES score way,large dose of methyl prednisolone group before the treatment no significant change(P 〉0.05). While the control group in the treatment of 3 d,5 d,7 dfes score significantly higher(P〈 0.05). And with a low dose of methyl prednisolone group,high-dose methyl prednisolone comparative difference was statistically significant(P〈 0.05). Were FES5 example,three groups of low doses of methyl prednisolone group in 1 case(3.13%),high doses of methyl prednisolone group 0 examples(0.0%) and control group in 4 cases(12.5%),the control group was significantly higher incidence of FES low doses of methyl prednisolone group,highdose methyl prednisolone group(P 〈0.05). Low doses of methyl prednisolone group,high-dose methyl prednisolone group there was no statistically significant difference comparing the incidence of FES(P〉 0.05). Conclusion:low dose can prevent lower limbs long bone fracture patients with post-traumatic pulmonary fat embolism.
