摘要 目的观察LARS人工韧带重建喙锁韧带的生物相容性并评价其近期疗效,为临床肩锁关节脱位治疗提供依据。方法选择本院2013年1月-2015年1月肩锁关节脱位患者30例,按随机数表法随机分为对照组和观察组,每组各15例。对照组采取锁骨钩钢板内固定手术,观察组采用LARS人工韧带进行重建喙锁韧带。患者均于伤后2-5天内在全麻下进行手术,观察并记录两组患者术后功能恢复情况及伤口感染、异物感、再脱位及疼痛感等指标。结果观察组复位不佳1例,占6.67%;对照组伤口感染1例,感染率为6.67%,异物感3例,占2P〈0.00%,复位不佳1例,占6.67%。观察组并发症出现概率低于对照组,差异具有统计学意义(P〈P〈0.05)。治疗后观察组患者UCLA评分为(25.29±P〈0.03)分,VAS评分为(1.59±P〈0.03)分;对照组患者UCLA评分为(27.29±P〈0.08)分,VAS评分为(2.79±P〈0.01)分。治疗后两组患者VAS评分差异有统计学意义(P〈P〈0.05),两组患者的UCLA评分差异无统计学意义(P〉P〈0.05)。采用Lazzcano标准评价患者临床疗效,观察组优良率为93.33%;对照组优良率为73.33%,观察组的优良率明显高于对照组,差异具有统计学意义(P〈P〈0.05)。结论 LARS人工韧带重建喙锁韧带具有良好的生物相容性,术后并发症发生率较低,值得在临床上推广使用。 Objective To observe the LARS artificial ligament reconstruction beak lock ligament biocompatibility and evaluate its recent curative effect, provide the basis for clinical treatment of acromioclavicular joint dislocation. Methods30 patients with acromioclavicular joint dislocation were chosen in our hospital from January 2013 to January 2015, according to the stochastic indicator method were randomly divided into control group and observation group, each group(n=15). Control group take clavicular hook plate internal fixation surgery, observation group with LARS artificial ligament reconstruction beak lock ligaments. Patients were within 2 to 5 day after injury surgery under general anesthesia, observe and record two groups of patients with postoperative function recovery and wound infection, foreign body sensation, and dislocation, pain index, etc. Results Observation group reduction and poor in 1 case, accounting for 6.67%; Control wound infection in 1 case, the infection rate was 6.67%, 2P〈0.00%, foreign body sensation in 3 reduction and poor in 1case, accounting for 6.67%. Observation group complications probability is lower than the control group, the difference statistically significant( P〈0.05). Observation group of patients after treatment for UCLA score(25.29±P〈0.03),(1.59±P〈0.03)for VAS score points; Control patients for UCLA score(27.29±P〈0.08),(2.79±P〈0.01) for VAS score points. Two groups of patients after treatment VAS score difference was statistically significant( P〈0.05), two groups of patients at the UCLA score difference has no statistical significance( P〈0.05). Adopt Lazzcano standard evaluation of patients with clinical curative effect of observation group was 93.33%; The control group was 73.33%, observation group was significantly higher than the control group, difference has statistical significance( P〈0.05). Conclusion LARS artificial ligament reconstruction beak lock ligament has good biological compatibility, lower incidence of postoperative complications, is worth popularizing in clinical use.
