摘要
目的探讨关节镜下外侧支持带松懈、内侧紧缩并髌韧带止点移位治疗复发性髌骨脱位的临床效果。方法以本院收治的64例单侧复发性髌骨脱位患者为观察对象,该组患者均采取关节镜下外侧支持带松懈、内侧紧缩并髌韧带止点移位治疗,比较治疗前后Q角、髌骨-股骨滑车适合角、髌股外侧角和股骨髁滑车凹中心与胫骨结节水平距离(TT-TG)。结果术后随访1年,未见复发病例,治疗后髌骨-股骨滑车适合角、髌股外侧角、TT-TG为(-2.2±5.8)°、(10.8±3.8)°、(13.5±1.7)mm,治疗后男性Q角(9.3±2.7)°,女性Q角(15.3±4.2)°,上述指标同治疗前比较差异有统计学意义;治疗后Lysholm评分为(92.6±10.6)分,高于治疗前。结论关节镜下外侧支持带松懈、内侧紧缩并髌韧带止点移位治疗复发性髌骨脱位的疗效确切,明显改善膝关节功能。
Objective To investigate effect of arthroscopic lateral retinaculum lax, medial tightening and patellar ligament insertion point displacement in the treatment of recurrent dislocation of the patella. Methods 64 cases of unilateral recurrent patellar dislocation were selected as object who admitted to our hospital. They were treated by arthroscopic lateral support with lax, medial tightening and patellar ligament insertion point displacement. Q angle, patella femoral block suitable for angle, lateral patellofemoral angle and femoral trochlea fovea and the tibial tuberosity horizontal distance（TT-TG） of before and after treatment were then compared. Results Follow-up period of 1 year, no recurrence was found. After the treatment of patella-femoral trochlea suitable angle, lateral patellofemoral angle, TT-TG were（-2.2±5.8） °,（10.8±3.8） °,（13.5±1.7） mm. After treatment, male Q angle was（9.3±2.7） °, the female Q angle was（15.3±4.2） °. The difference of these indicators had statistical significance. Lysholm score after treatment was（92.6±10.6） scores, which significantly higher than the before treatment. Conclusion Arthroscopic lateral retinaculum lax, medial tightening and patellar ligament insertion point displacement are effective in the treatment of recurrent dislocation of the patella effect, which significantly improve the function of knee joint.
