降钙素原和急性生理与慢性健康评分Ⅱ对脓毒症患者预后的评估 被引量:12

摘要 目的探讨降钙素原(PCT)与急性生理与慢性健康评分Ⅱ(APACHEⅡ)评分预测脓毒症患者预后的作用。方法选取2013年4月至2015年7月黄河三门峡医院收治的60例脓毒症患者,入住重症医学科(ICU)和急诊监护室(EICU)后第1、3、7天分别监测血清PCT水平的动态变化,同时记录其APACHEⅡ评分情况。根据患者28 d生存情况将其分为死亡组和生存组,比较2组患者血清PCT水平及APACHEⅡ评分。结果死亡组患者入住ICU或EICU后第1、3、7天血清PCT水平及APACHEⅡ评分比较差异无统计学意义(P>0.05)。生存组患者入住ICU或EICU第7天血清PCT水平和APACHEⅡ评分显著低于第1天(P<0.05)。死亡组患者入住ICU或EICU后第1、3、7天血清PCT水平和APACHEⅡ评分均显著高于生存组(P<0.05)。结论血清PCT水平及APACHEⅡ评分可用于评估脓毒症患者的预后,2个指标持续高水平提示预后不良。 Objective To explore the effect of procalcitonin( PCT) and acute physiology and chronic health evaluationⅡ( APACHEⅡ) scores on predicting prognosis of sepsis patients. Methods Sixty patients with sepsis were selected in Huanghe Hospital of Sanmenxia City from April 2013 to July 2015. The change of serum PCT was detected at the first,third and seventy day after they staying in intensive care unit( ICU) or emergency intensive care unit( EICU),and the APACHEⅡscore was recorded. The patients were divided into death group and survival group according to the 28-day survival situation and the level of PCT and APACHEⅡ score were compared between the two groups. Results There was no significant difference in the level of PCT and APACHEⅡ score in death group among the time at the first,third and seventy day after staying in ICU or EICU( P〈0. 05),while the level of PCT and APACHEⅡ score in survival group at the seventh day after staying in ICU or EICU were lower than those at the first day after staying in ICU or EICU( P〈0. 05). The level of PCT and APACHEⅡscore in death group at the first,third and seventy day after staying in ICU or EICU were higher than those in survival group( P〈0. 05). Conclusion PCT and APACHEⅡ score could be used to predict prognosis of sepsis patients,it hints that the prognosis is poor if the two indexes increase.
