改良股骨远端前外侧入路内、外双侧锁定钢板治疗老年股骨远端C型骨折的疗效 被引量:10

The Treatment Effect of Double Locking Plates with Modified Anterolateral Approaches on Distal Femur Type C Fractures

摘要 目的总结改良股骨远端前外侧入路内、外侧双锁定钢板治疗老年股骨远端C型骨折的临床疗效。方法采用改良股骨远端前外侧入路单一切口显露股骨远端,股骨外侧应用锁定钢板,内侧应用支持钢板治疗老年股骨远端粉碎骨折并获得随访患者11例,其中男2例,女9例;年龄69~82岁,平均73.1;AO分型:C1型8例,C2型3例,C3型0例。结果本组11例患者,平均随访12.4个月,骨折愈合时间19~28周,平均22.5周。术后6~8个月,骨折愈合后,采用美国特种外科医院膝关节评分标准评价膝关节功能。优2例,良6例,可3例,优良率为72.7%。结论老年股骨远端C型粉碎骨折采用改良股骨前外侧单一入路,行内、外侧锁定钢板治疗,可以避免对股骨内侧软组织的损害,充分显露股骨远端,并做坚强固定,早期功能锻炼,获得较好临床效果。 Objective To analyze the treatment effect of double locking plates with modified anterolateral approaches on elder distal femur comminuted fracture. Methods Eleven cases(2 male, 9 female) were treated with modified anterolateral approaches. Average age was 73.1(range:69-82). Locking plate was used on lateral femur, and supporting plate was used on medial femur. AO classification: 8 of C1, 3 of C2, 0 of C3. Results The mean follow-up was 12.4months and the average healing time was 22.5 weeks. The excellent and good rate of patients' HSS at half year after operation was 72.7%. Conclusion Double locking plates with modified anterolateral approaches for distal femur comminuted fracture could avoid the injury of soft tissue in medial femur, and make the fracture fixed strongly to facilitate early exercise.
