摘要
目的:观察加味四妙勇安汤治疗糖尿病足下肢溃疡的疗效及其对红细胞沉降率(ESR)、超敏C反应蛋白(hs-CRP)、基质金属蛋白酶-9(MMP-9)的影响。方法:将86例糖尿病足患者随机分为观察组和对照组,每组43例,两组均给予控制血糖、控制感染、改善下肢循环及常规伤口换药治疗;在此基础上,观察组加服加味四妙勇安汤,对照组加服西洛他唑片。比较两组疗效及治疗前后ESR、hs-CRP、MMP-9水平差异。结果:观察组总显效率(81.40%)明显高于对照组(60.47%),差异有统计学意义(P<0.05),但观察组有效率(93.02%)与对照组(86.05%)差异无统计学意义(P>0.05)。与治疗前比较,两组治疗4周后和治疗8周后ESR、hs-CRP水平均下降,治疗8周后下降更明显,差异有统计学意义(P<0.01);治疗前和治疗4周后,观察组与对照组ESR、hs-CRP水平的组间差异无统计学意义(P>0.05),治疗8周,观察组与对照组ESR水平差异无统计学意义(P>0.05),但观察组hs-CRP水平下降较对照组更明显(P<0.01)。与治疗前比较,对照组治疗4周后MMP-9水平无明显下降(P>0.05),治疗8周后显著下降(P<0.01);观察组治疗4周后和治疗8周后MMP-9水平均下降(P均<0.01),治疗8周后较治疗4周后下降更明显(P<0.05)。治疗前,观察组与对照组MMP-9水平差异无统计学意义(P>0.05);治疗4周后和治疗8周后,观察组较对照组明显下降(P<0.05)。治疗期间两组均未出现严重不良反应。结论:加味四妙勇安汤治疗糖尿病足临床疗效优于西医治疗。
Objective：To observe the clinical curative effect of Jiawei Si Miao Yong An decoction in the treatment of diabetic foot ulcer of lower limb and the influence on Erythrocyte Sedimentation Rate（ESR）,high sensitive C reactive protein（hs-CRP）and matrix metalloproteinase 9（MMP-9）of peripheral blood in patients.Method：86patients with diabetic foot ulcer of lower limb were randomly divided into observation group and control group,each group had 43 cases.On the basis of routine treatment which including control blood glucose,control infection,improve lower extremity circulation and conventional wound dressing treatment,the observation group was given the treatment of Jiawei Si Miao Yong An decoction and the control group was given cilostazol tablets.The curative effect of the two group and serum levels of ESR,hs-CRP,MMP-9before and after treatment were analyzed.Results：The total effective rate in the observation group（81.40%）was significantly higher than that in the control group（60.47%）,the difference was statistically significant（P〈0.05）,but the effective rate in the observation group（93.02%）and the control group（86.05%）was not statistically significant（P〉0.05）.Compared with before treatment,the levels of ESR and hs-CRP of two groups decreased both after treatment for 4weeks and 8weeks,and decreased more obviously after treatment for 8weeks,the difference was statistically significant（P〈0.01）;the levelsof ESR,hs-CRP of two groups were not statistically significant before treatment and after 4weeks treatment（P〉0.05）,the levels of ESR of two groups were no significant difference after 8weeks treatment（P〉0.05）,but the levels of hs-CRP in the observation group was significantly lower than that in the control group（P〈0.01）.Compared with before treatment,the levels of MMP-9of the control group had no significant decrease after 4weeks treatment（P〉0.05）,but decreased after 8weeks treatment（P〈0.01）;the levels of MMP-9of the observation group decreased after treatment for 4 weeks and 8 weeks（P〈0.01）,and decreased more obviously after treatment for 8weeks compared with treatment for 4weeks（P〈0.05）;the levels of MMP-9of two groups were not statistically significant before treatment（P〉0.05）;but the levels of MMP-9in the observation group was significantly lower than that in the control group after treatment for 4weeks and 8weeks（P〈0.05）.No serious adverse reactions were found in the two groups during the treatment.Conclusion：Jiawei Si Miao Yong An decoction is effective than western medicine treatment in the treatment of diabetic foot of lower limb.
