气管插管用引导管芯在困难气道气管插管术中的应用 被引量:4

摘要 目的评价一次性气管插管用引导管芯(国家专利)用于困难气道患者气管插管术的效果。方法择期行气管插管全麻下手术患者60例,男49例,女11例,年龄20~65岁,体质指数21.2~28.6 kg/m^2,ASA分级Ⅰ或Ⅱ级,Cormack&Lehane分级Ⅲ或Ⅳ级。采用随机数字表法,将患者随机分为两组(n=30):C组和D组。C组采用普通管芯引导气管插管;D组采用气管插管引导用管芯(专利产品)引导气管插管。记录诱导后插管前(T0)、插管后1 min(T1)、3 min(T2)及5 min(T3)时平均动脉压、心率、脉搏氧饱和度;记录插管时间、插管次数及气管插管相关并发症;计算首次气管插管成功率及气管插管总成功率。结果与T0时比较,两组患者T1~T2时平均动脉压升高、心率增快(P<0.05);与T1时比较,两组患者T3时平均动脉压降低、心率减慢(P<0.05);两组患者各时间点平均动脉压、心率、脉搏氧饱和度组间比较差异无统计学意义(P>0.05);与C组比较,D组气管插管时间减少、插管次数减少、首次插管成功率和插管总成功率均增加(P<0.05),患者口咽黏膜出血及气管导管拔出后24h内咽喉痛发生率均降低(P<0.05)。结论一次性气管插管用引导管芯可提高困难气道气管插管成功率。
