摘要
目的比较腹腔镜与开腹2种术式对右半结肠癌患者免疫功能的影响。方法选取右半结肠癌需行手术患者100例,根据手术方法不同分为腹腔镜组和开腹组,各50例,比较2组患者手术前后C反应蛋白(CRP)、白细胞(WBC)、白细胞介素(IL)-6、CD_4^+、CD_8^+、CD_4^+/CD_8^+和淋巴细胞数(Lymph)的变化情况。结果与手术前比较,2组患者WBC、CRP、IL-6均明显升高,差异有统计学意义(P<0.05或P<0.01),术后2组比较CRP、IL-6、CD_4^+、CD_8^+差异有统计学意义(P<0.05);腹腔镜组腹腔引流液WBC、IL-6低于开腹组,差异有统计学意义(P<0.05)。结论腹腔镜右半结肠根治术对患者机体免疫功能影响较小,术后炎症反应低。
Objective To compare the influence of laparoscope and laparotomy on the immune function of patients with right-half colon cancer. Methods A total of 100 patients with right-half colon cancer who required surgery were divided into laparoscope group and laparotomy group according to different surgical methods, with 50 cases in each group. Changes of C-reactive protein （CRP） , white blood cell count （WBC）, interleukin （IL）-6, CD4^＋, CD8^＋, CD4^＋/CD8^＋ and lymphocyte count （Lymph） were compared between two groups before and after surgery. Results Compared with surgery before, WBC, CRP and IL-6 increased evidently in both groups after surgery, and the differences were significant （P 〈 0.05 or P 〈 0.01 ）, and there were also significant differences between two groups in CRP, IL-6, CD4^＋ and CD8^＋ after surgery （P 〈 0.05）. Laparoscope group was prominently lower than laparotomy group in the levels of WBC and IL-6 in peritoneal drainage fluid, and there was significant differences （P 〈 0.05 ）. Conclusion Laparoscope-guided radical surgery of right-half colon cancer has less influence on patients＇immune function and lower postoperative inflammatory responses.
