摘要
目的探讨甲状腺癌颈淋巴结转移诊断中D2—40免疫组化检测和彩超的诊断效果。方法选取2013年1月~2016年1月广东省佛山市第二人民医院收治的40例甲状腺癌患者设为A组,40甲状腺腺瘤患者设为B组,40例甲状腺组织正常者设为对照组。所有患者均进行D2—40免疫组化检测和彩超诊断,对比三组患者诊断效果。结果手术病理检查,40例甲状腺癌患者颈淋巴结转移率为100.O%。超声诊断准确率、灵敏度、特异性与术前触诊相比明显较高(P〈0.05)。A组患者颈淋巴结密度与B组、对照组相比明显较高(P〈0.05)。结论临床上彩超诊断甲状腺癌颈淋巴结转移诊断准确率高,甲状腺癌患者D2—40免疫组化染色检测时患者颈淋巴结密度明显高于甲状腺良性病变患者,临床可采取D2—40免疫组化染色辅助诊断甲状腺癌颈淋巴结转移诊断,提高诊断效果。
Objective To explore the diagnostic effect of D2-40 immunohistochemistry assay and color Doppler ultrasound in the diagnosis of cervical lymph node metastasis of thyroid carcinoma. Methods 40 cases of thyroid cancer patients were divided into group A, 40 cases of thyroid adenoma patients were divided into group B, and 40 cases of normal thyroid tissue person were divided into control group. All the patients were selected in the second people' s Hospital of Foshan City, Guangdong Province from January 2013 to January 2016. All the patients were examined with D2-40 immunohistochemistry assay and color Doppler ultrasound diagnosis. The diagnostic effects of the three groups were compared. Results By surgical pathological examination, cervical lymph node metastasis rate of group A was 100.0%. Compared with the preoperative palpation, the ultrasound diagnostic accuracy, sensitivity, specificity and preoperative palpation was significantly higher(P 〈 0.05). Neck lymph node density of group A was significantly higher than that of group B and control group(P 〈 0.05). Conclusion The clinical accuracy rate of color Doppler ultrasound in the diagnosis of cervical lymph node metastasis of thyroid carcinoma was high. Neck lymph node density of thyroid cancer patients during the D2-40 immunohistochemical staining was significantly higher than that of patients with benign thyroid disease. The D2-40 immunohistochemical staining could help to diagnose cervical lymph node metastasis of thyroid carcinoma to improve the diagnostic effect.
