摘要
目的探讨2型糖尿病患者发生早期肾病的危险因素。方法 选择2015年1月~2016年3月在本院住院的2型糖尿病患者229例,根据尿微量白蛋白/肌酐比值(UACR)分为DM组(UACR〈30 mg/g)158例和早期糖尿病肾病组(30 mg/g≤UACR≤300 mg/g)71例。收集患者基本临床资料,对常规实验室指标进行检测,利用统计学软件对上述指标进行分析。结果 早期糖尿病肾病(DN)组的年龄、病程、收缩压、CRP、SA、Cr、FIB、DD均显著高于DM组(P均〈0.05),而HGB显著低于DM组(P〈0.05),其余指标比较差异均无统计学意义(P均〉0.05)。多因素Logistic回归分析显示年龄、收缩压和血清唾液酸(SA)为糖尿病患者发生早期肾病的独立危险因素。三者联合应用诊断早期DN的ROC曲线下面积(AUC)达到0.73(0.66~0.80)(P〈0.01)。结论 年龄、收缩压和血清SA是2型糖尿病患者并发早期DN的危险因素,临床应密切关注其变化。
Objective To investigate the risk factors in type 2 diabetes patients with early nephropathy. Methods A to- tal of 229 cases of type 2 diabetic patients from January 2015 to March 2016 in our hospital were enrolled. According to urinary albumirdereatinine ratio (UACR), patients were divided into DM group (UACR〈30 mg/g) with 158 cases and early diabetes nephropathy group (30 mg/g ≤ UACR ≤ 300 mg/g) with 71 cases. The basic clinical data of patients were collected, and routine laboratory tests were detected. The statistical software was applied to analyze these indicators. Results Age, duration, systolic blood pressure, CRP, SA, Cr, FIB, DD in early diabetic nephropathy(DN) group were significantly higher than those of DM group (all P〈O.05), but HGB in early diabetic nephropathy(DN) group was lower than that of DM group (P〈0.05). The differences in other indicators were not statistically significant between the two groups(all P〉O.05). Multivariate Logistic regression analysis showed that the independent risk factors were age, systolic blood pressure and serum sialic acid (SA) in diabetic patients with early diabetic nephropathy. The area under the ROC curve (AUC) of the three indicators combined for the early diagnosis of DN reached 0.73 (0.66-0.80)(P〈0.01). Conclusion Age, systolic blood pressure and serum SA are risk factors for early DN in type 2 diabetes patients, and clinic should pay close attention to the changes of these markers.
