摘要
目的探讨老年慢性心力衰竭(心衰)患者N-末端B型脑利钠肽前体(NT-Pro BNP)、超敏C反应蛋白(hsCRP)、白细胞介素-10(IL-10)水平与心功能的相关性。方法选取我院收治的老年慢性心衰患者79例为试验组,同时选取45名健康体检志愿者为对照组,分别应用多普勒超声检查、酶联免疫吸附法、免疫比浊法和胶体金法对两组受试者心脏收缩功能、IL-10、hs-CRP及NT-Pro BNP进行检测。结果试验组超声检查显示有35例心脏收缩功能正常,44例收缩功能不全;其中收缩功能正常组和不全组NT-Pro BNP、hs-CRP均显著高于对照组,IL-10水平显著低于对照组,3组间比较差异有统计学意义(P<0.05);试验组不同心功能分级患者NT-Pro BNP、hs-CRP均明显高于对照组,IL-10水平低于对照组,4组间比较差异有统计学意义(P<0.05);NT-Pro BNP、hs-CRP与心功能分级呈正相关(P<0.05),IL-10与心功能分级呈负相关(P<0.05)。结论老年慢性心衰患者心功能和NT-Pro BNP、hs-CRP、IL-10水平存在明显相关性,可作为临床诊断和病情评估的重要指标。
Objective To study the clinical correlations between plasma NT-ProBNP,hs-CRP and serum IL-10 levels and the heart function in elderly patients with chronic heart failure. Methods 79 elderly patients with chronic heart failure treated in this hospital from November 2014 to December 2015 were recruited as study group,another 45 healthy volunteers as control group. The cardiac function,IL-10,hs-CRP and NT-ProBNP indexes of patients in the two groups were detected by Doppler ultrasonography,enzyme linked immunosorbent assay,immunoassay and colloidal gold methods. Results In the study group,35 patrents had normal cardiac systolic function,and the other 44 patients had systolic dysfunction. The NT-ProBNP,hs-CRP levels in the normal cardiac systolic function group and systolic dysfunction group were significantly higher than those in the control group,while the IL-10 level was significantly lower; the difference was statistically significant between the 3 groups( P〈0. 05). The NT-ProBNP and hs-CRP levels of different cardiac function classification in the study group were significantly higher than those in the control group,while the IL-10 level was significantly lower; the difference was statistically significant between the 4 groups( P〈0. 05). The NT-ProBNP and hs-CRP levels were positively correlated with the cardiac functional grading( P〈0. 05),while there was a negative correlation between the IL-10 level and the cardiac functional grading( P〈0. 05). Conclusions The levels of plasma NT-ProBNP,hs-CRP and serum IL-10 are obviously correlated with cardiac function in elderly patients with chronic heart failure,which can be used as an important indicator for clinical diagnosis and disease evaluation.
