摘要
目的：系统评价集束化护理在 ICU 患者压疮（pressure ulcer，PU）管理中的应用效果。方法应用计算机对2016年3月前发表的国内外相关文献进行检索，收集集束化护理策略在ICU压疮管理中应用的随机对照试验，并利用 Stata 12.0软件进行处理分析。结果符合纳入标准的文献共16篇，3027例患者。 Meta分析显示，集束化护理有助于降低 ICU患者压疮发生率[OR＝0.31，95％CI （0.25，0.38）]、延长压疮发生时间[WMD＝3.56，95％CI（3.05，4.06）]。敏感性分析证实结局指标合并结果可信，漏斗图显示本研究不存在发表偏倚。结论集束化护理在降低ICU患者压疮发生率、延长难免压疮发生时间方面优于常规护理措施，建议临床推广应用。
Objective To evaluate systematically the application effect of cluster nursing on pressure ulcer （PU） management of ICU patients.Methods Randomized control trials （RCTs） relevant to application of cluster nursing strategy in ICU PU management were collected by searching related Chinese and foreign literature published before March 2016, with the results analyzed by Stata 12. 0. Results 16 articles with 3 027 patients involved were in accordance with the criteria. It was shown in the Meta analysis that cluster nursing helped to reduce incidence of PU of ICU patients [OR=0.31,95%CI（0.25,0.38）] and to postpone occurrence of PU[WMD= 3.56, 95%CI（3.05,4.06）]. Results with all outcome indicators combined were confirmed reliable by sensitivity analysis. No existence of publication bias was shown in funnel plot. Conclusions Compared with routine nursing measures, cluster nursing showed superiority in reducing incidence rate and postponing occurrence time of PU of ICU patients, which makes it worth promoting in clinical practice.
