摘要
目的:探讨微波消融治疗在肝癌患者中的临床应用。方法:选择69例接受微波消融治疗的肝癌患者,根据患者最大肿瘤直径大小将其分为小肝癌组(肿瘤直径≤5 cm,43例)和大肝癌组(肿瘤直径5~8 cm,26例),对两组患者均行微波消融治疗,观察两组患者术前、术后肝功变化,比较两组患者术后并发症、住院时间、复发率、新发率及生存率情况。结果:经微波消融治疗1周后,小肝癌组血清丙氨酸转氨酶(ALT)低于大肝癌组,两组比较有差异(t=-2.06,P<0.05);小肝癌组白蛋白(ALB)和胆碱酯酶(CHE)高于大肝癌组,两组比较有差异(t=4.13,t=5.54;P<0.05);小肝癌组患者的并发症少,住院时间短,随访6~24个月,小肝癌组复发和新发率明显低于大肝癌组,小肝癌组生存率高于大肝癌组,两组比较有显著差异(x^2=10.42,x^2=18.01;P<0.01)。结论:微波消融治疗小肝癌效果较大肝癌效果好,对于大肝癌的微波消融治疗应寻求多种治疗方式联合,从而提高疗效。
Objective To investigate the clinical application of microwave ablation （MWA）in the treatment of liver cancer. Methods： A total of 69 patients diagnosed with liver cancerand received microwave ablation in our hospital from June 2012 to March 2014 were selected.According to their maximum tumor diameter size, they were divided into small hepatocellularcarcinoma group with 43 patients and large hepatocellular carcinoma group with 26 cases. All the patients underwentmicrowave ablation therapy. Changes in liver function before and after surgery, postoperative complications, lengthof stay, recurrence rate, new-onset cases and survival rate of patients were observed. Results： One week after MWAtreatment, the ALT in small HCC group was less than large HCC group, and the difference was significant （t=-2.06,P〈0.05）. The ALB and CHE were higher, and the differences were significant （t=4.13, t=5.54; P〈0.05）. Small HCCgroup had fewer complications and shorter hospital stay. With 6-24 months followed up, the recurrence rate, new-onsetcases and survival rate in small HCC group were less than the large HCC group, and the two groups were significantlydifferent （x2=10.42, x2=18.01; P〈0.01）. Conclusion： MWA for small hepatocellular carcinoma has better effect thanlarge hepatocellular carcinoma. For large hepatocellular carcinoma, a variety of treatment modalities should be used toimprove the efficacy.
