维生素D辅助治疗对多囊卵巢综合征内分泌代谢相关指标及胰岛素抵抗的影响 被引量:7

Effect of Related Indicators and Insulin Resistance of Endocrine Metabolism in Vitamin D Supplement Adjuvant Treatment for Polycystic Ovary Syndrome

摘要 目的:探讨维生素D辅助治疗对多囊卵巢综合征内分泌代谢相关指标及胰岛素抵抗的影响。方法:多囊卵巢综合征(PCOS)患者74例,随机分为观察组和对照组(各37例)。对照组采用基础治疗方法,观察组在对照组的基础上另予维生素D口服,1200IU/d。比较两组治疗前后促卵泡激素(FSH)、促黄体生成素(LH)、睾酮(T)、25-羟基维生素D3(25-OH-VD3)等内分泌代谢指标水平及胰岛素抵抗发生率。结果:经治疗后观察组卵泡刺激素水平[(5.86±1.64)mIU/mL]略高于对照组[(5.74±1.45)mIU/mL],但差异无统计学意义(P>0.05);促黄体生成素、睾酮水平及胰岛素抵抗率[(4.27±1.42)mIU/Ml]、(34.65±8.27)nmol/L、18.92%]均低于对照组[(6.35±1.74)mIU/mL、(40.54±9.86)nmol/L、43.24%],差异具有显著性(P<0.05);25-羟基维生素D3浓度[(35.62±6.51)nmol/L]高于对照组[(32.45±5.87)nmol/L],差异有统计学意义(P<0.05)。结论:维生素D补充辅助治疗可以通过提高患者血清中25(OH)VD3浓度来降低PCOS患者的睾酮及促黄体生成素水平,控制患者的内分泌代谢紊乱,降低PCOS患者胰岛素抵抗的发生率。 Objective:To discuss the effect of related indicators and insulin resistance of endocrine metabolism in Vitamin D supplement adjuvant treatment for polycystic ovary syndrome.Methods:74cases of hospitalized patients with polycystic ovary syndrome(PCOS)from February 2013 to February 2015 in a hospital were randomly divided into observation group(37cases)and control group(37cases).Foundation treatment method was used for the control group and observation group was gievn oral vitamin D(1200iu/D)treatment on the basis of the control group.Then observed and compared the endocrine metabolism index levels such as FSH,LH,T and 25-OH-VD3,and the incidence rate of insulin resistance of two groups before and after treatment.Results:The FSH level of observation group [(5.86±1.64)mIU/ml]was slightly higher than that of control group[(5.74±1.45)mIU/ml]after treatment,and the difference was not statistically significant(P〉0.05);LH [(4.27±1.42)mIU/ml],T[(34.65±8.27)nmol/L]and the incidence rate of insulin resistance(18.92%)of observation group were significantly lower than those of control group[(6.35±1.74)mIU/ml,(40.54±9.86)nmol/L,43.24% ],and the difference was statistically significant(P 0.05);The concentration of 25-OH-VD3 in observation group[(35.62±6.51)nmol/L]was significantly higher than that of control group[(32.45±5.87)nmol/L],and the difference was statistically significant(P〈0.05).Conclusion:The vitamin D adjuvant treatment can improve 25(OH)VD3concentration in patients serum to reduce the testosterone and luteinizing hormone levels of patients with PCOS,which can control endocrine metabolic disorders of patients,and reduce the incidence rate of insulin resistance in PCOS patients.
